Develop, coordinate, and maintain education on Waiver Incident Reporting from start to finish.
Identify and process reported waiver incidents in alignment with state/federal regulations (e.g., abuse, neglect, misappropriation, exploitation, health and welfare).
Oversee Waiver Incident Management with state-contracted investigation organizations.
Investigate waiver incidents not handled by contracted investigation organizations.
Determine and resolve prevention plans.
Audit Waiver Incidents to ensure compliance for annual state/federal audits.
Collect and review clinical documentation to determine outcomes of “Death or Unexplained Death.” If unexplained, escalate to an external agency for further review.
Perform other duties as assigned.
Comply with all policies and standards.
Requirements:
Degree in Nursing, Social Work, and/or Counseling.
Five years of experience in relevant fields.
Knowledge of LTSS, Waiver, and Incident Reporting.
RN, LSW, LISW, LPC, or LPCC certification required.
Preferred Skills:
Bachelor’s degree in Nursing and Master’s in Social Work and/or Counseling.
