Job description

**Job Title:** Haitian Creole Care Coordination Specialist
**Location:** Remote (Home-based in Haiti only)
**Employment Type:** Full-Time (40 hours/week)
**Work Schedule:** 9:00 AM to 5:00 PM EST (flexible if needed)

**Position Overview:**
We are seeking an individual freelancer (not an agency or BPO) based in Haiti to join our team as a Care Coordination Specialist. The specialist will support Haitian Creole-speaking members and caregivers through outbound calls, intake follow-ups, and new referral onboarding. This role is essential in reinforcing program expectations, confirming daily visits, addressing no-shows, and performing compliance follow-ups for at-risk members. All interactions must be documented in the CRM system, with SMS or WhatsApp follow-ups sent as appropriate.

**Key Responsibilities:**
- Conduct outbound calls to Haitian Creole-speaking members and caregivers
- Assist with intake follow-ups and new referral onboarding
- Reinforce program expectations and visit schedules
- Confirm daily visits and address no-shows
- Perform compliance follow-ups for at-risk members
- Document all interactions in the CRM system

**Required Qualifications:**
- Fluent in Haitian Creole, French, and English (verbal and written)
- Prior experience in call center, customer service, or patient coordination roles
- Comfortable with high call volumes and structured workflows
- Strong communication and documentation skills

**Preferred Qualifications:**
- Experience in healthcare, home care, or Medicaid-related programs
- Familiarity with CRM systems
- Experience working with remote teams

**Additional Preferences:**
- Reliable and organized
- Comfortable in a fast-paced, process-driven environment
- Experience supporting underserved or diverse populations is a plus
- Stable internet connection and a backup generator required
