This is a hybrid/remote role and would need to live in the state of IL

  • Coordinate day-to-day coordination of the TMaH project across IL Condell and Sherman Advocate sites by ensuring alignment with approved workplans and grant requirements.
  • Serve as the primary on-site and operational liaison among internal clinical, community, operational, finance, and analytics teams.
  • Coordinate engagement with community-based organizations, doula partners, and other external stakeholders supporting Medicaid populations.
  • Track, manage, and support completion of all grant deliverables across TMaH pillars including access/workforce, quality and safety, and whole-person care.
  • Identify risks to timelines, deliverables, or compliance and escalate issues with recommended mitigation strategies.
  • Ensure documentation and evidence are maintained to support audits, monitoring, and performance reviews.
  • Support sustainability and scalability planning for successful interventions beyond the grant period.

Supports broader maternal health bundle strategy.

Pay Range

$41.10 - $61.65

Major Responsibilities:

  • Serve as the primary on-site and operational liaison among internal clinical, community, operational, finance, and analytics teams.
  • Coordinate engagement with community-based organizations, doula partners, and other external stakeholders supporting Medicaid populations.
  • Track, manage, and support completion of all grant deliverables across TMaH pillars including access/workforce, quality and safety, and whole-person care.
  • Maintain master project plans, timelines, and deliverable trackers aligned to quarterly and annual reporting requirements.
  • Coordinate preparation and submission of all required reports in coordination with the Foundation team, including monthly expense reports, quarterly milestone reports, and final grant reporting.
  • Support site visits, stakeholder meetings, learning collaboratives, and funder-led calls or webinars.
  • Identify risks to timelines, deliverables, or compliance and escalate issues with recommended mitigation strategies.
  • Ensure documentation and evidence are maintained to support audits, monitoring, and performance reviews.
  • Support sustainability and scalability planning for successful interventions beyond the grant period.

Essential Functions:

1. Executes programs that meet the needs of the organization's target population and that are in alignment with the organization's stated mission.

2. Supervises the work of program staff to ensure the quality of service provided.

3. Assists in the development of customized solutions to individual client needs.

4. Participates in program evaluation by collecting and analyzing data in support of key performance metrics.

5. Ensures that all services provided are in compliance with applicable standards and regulations.

Licensure, Registration, and/or Certification Required:

None Required.

Education Required:Bachelor’s degree in Public Health, Healthcare Administration, Social Work, or a related field required, Project Management experience highly desirable.

Experience Required:

3–5 years of experience in project coordination, program management, or grant-supported initiatives within healthcare, public health, or government-funded programs. Experience working with Medicaid populations and community-based organizations strongly preferred.

  • Comprehensive knowledge of the field's policies, procedures, and practices. Strong analytical and critical thinking skills; demonstrated ability to learn and apply technical and regulatory rules and process large amounts of complex information.
  • Must be a skilled problem solver who can work independently and meet deadlines and work well under pressure.
  • Demonstrated management skills and ability to manage special projects, work under pressure, meet deadlines.
  • Customer service orientation, strong collaboration skills; verbal and written communication skills and presentation skills to effectively persuade and influence in order to achieve desired outcomes with diverse clientele; strong interpersonal skills to effectively build relationships with business partners across departments and facilitate work among teams.
  • Project management skills Ability to develop and mentor others.
  • Strong project management and organizational skills with ability to manage multiple concurrent deliverables and deadlines.
  • Demonstrated ability to coordinate across multidisciplinary internal teams and external partners.
  • Working knowledge of grant compliance, reporting requirements, and performance monitoring.
  • Excellent written and verbal communication skills, including preparation of formal reports and stakeholder updates.
  • Ability to work independently, anticipate needs, and proactively resolve issues.
  • Comfort operating in both on-site and system-level environments with frequent stakeholder interaction.
