Roles & Responsibilities

  • Licensed Master's or Ph.D. in an appropriate field (social work, psychology, mental health counseling)
  • Valid NYS Driver's License
  • Training and experience working with adolescents and/or young adults with serious mental illness
  • Experience in community-based mental health services

Requirements:

  • Serve as the primary clinician for a subset of participants and collaborate with families; provide community-based services, conduct needs and safety/wellness planning, connect families to supports, and oversee discharge planning with stable service linkage.
  • Deliver clinical interventions and psychoeducation, including Social Skills Training, Substance Abuse Treatment, Behavioral Activation, Coping Skills Training, psychoeducation on first episode psychosis (FEP), and supportive therapy; utilize shared decision-making to tailor treatment.
  • Conduct assessments of social functioning and substance use; coordinate treatment planning and transitions; ensure continuity of care, active participation in rotating on-call duties and weekly team meetings.
  • Outreach and Recruitment: establish relationships with community partners; conduct outreach and education to reduce inequities and barriers; screen referrals, maintain a database of referral sources, and ensure a welcoming intake process; refer ineligible cases to appropriate providers.

Job description

Pay $75,000 - $90,000 annually 

The OnTrackNY Program is a Coordinated Specialty Care (CSC) treatment program at Community Health and Behavioral Services serving adolescents and young adults who are impacted by unexpected changes in their thinking and perceptions. Our mission is to empower young people to make meaning of their experiences and to pursue their goals for school, work and personal relationships. OnTrackNY team staff receive expert training and technical assistance in the CSC model and protected time for professional development through the statewide OnTrackNY Initiative at the Center for Practice Innovations at Columbia Psychiatry and New York State Research Foundation for Mental Hygiene.

 

Core Responsibilities 

The Primary Clinician and Outreach and Recruitment Coordinator (PC/ORC) is a full-time position. Serves as the Primary Clinician for a subset of program participants and supports with Outreach and Recruitment duties.   Activities include, but are not limited to the following:

Primary Clinician (PC) Role, the duties include:

  • Serve as the point person for the participant and their family;
  • Provide services in the community (off-site) as needed, based on participant and family preferences.
  • Conduct needs assessments;
  • Engage in safety planning and wellness management planning with participants;
  • Connect participants and families with the services and supports they need;
  • Provide behavioral interventions and take an especially active role in the family support domain.
  • Conduct clinical assessment to determine the participant’s level of social functioning and substance use via a shared decision making (SDM) framework
  • Provide Social Skills Training, Substance Abuse Treatment, Behavioral Activation, Coping Skills Training, and Psycho-education.
  • Provide psychoeducation regarding first episode psychosis (FEP) and the goals of the OnTrackNY program;
  • Provide supportive therapy;
  • Develop and modify treatment using a shared decision-making process with clients
  • Work with participants and family for transition from the team to other services and supports and ensure stable links at discharge.
  • Rotating on-call responsibilities with other team members
  • Attending weekly team meetings

 

Outreach and Recruitment Coordinator (ORC) duties include:

  • Establishing warm and collaborative relationships with community partners who might refer or provide ongoing support to young people and families
  • Spreading awareness about OnTrackNY services through community education and outreach activities that emphasize OnTrackNY’s uniquely flexible and youth-friendly approach
  • Collaborate with OnTrackNY team members to set new community-based goals focused on reducing inequities and barriers to accessing specialty care
  • Identifying and creating new resources to support diverse young people and families seeking or receiving services at OnTrackNY
  • Ensuring a welcoming and efficient process for young people and families seeking care in OnTrackNY
  • Serving as primary contact person for referral sources and maintaining on-going communication with referral sources
  • Conducting telephone and in-person eligibility screening of young people referred to OnTrackNY
  • Referring participants ineligible for services to other appropriate community providers
  • Establishing and maintaining database of referral sources with up-to-date contact information

 

Required Qualifications

  • Licensed Masters or Ph.D. in an appropriate field (social work, psychology, mental health counseling).
  • Valid NYS Driver's License. 

 

Preferred Qualifications

  • Training and experience working with adolescents and/or young adults with serious mental illness.
  • Experience in community-based mental health services
