Position Overview

The ImpactAdvocate Clinical Risk Analyst will be responsible for management and status reporting related to members that are at-risk of declining kidney health or kidney failure. This role will provide oversight to members with late-stage CKD and monitor their approach to dialysis. Appropriate escalations will be made to the ImpactAdvocate nursing team and the ImpactProtect team. This role will be responsible for predictive reporting and providing insights on the health of the risk pool.

How You’ll Contribute

Member Risk Assessment & Oversight
• Lead comprehensive risk assessments for all members in the clinical risk pool, including the collection and review of medical records, laboratory results, and accurate contact information.
• Evaluate member status and escalate cases to ImpactAdvocate nurses for advanced clinical review and risk stratification.
• Monitor member progression toward dialysis and identify opportunities for early interventions.

Risk Pool Management
• Serve as the primary owner of risk pool operations, ensuring accurate tracking, timely updates, and appropriate escalations to the ImpactProtect team.
• Complete and maintain Member Information Forms.
• Anticipate emerging risks and act proactively to mitigate potential adverse outcomes.

Departmental & Client Reporting
• Own and maintain all departmental risk reporting, including the Clinically Reviewed Forecast, Healthcare Navigation Dashboard, and client-facing performance reports.
• Translate complex data into actionable insights for leadership, nursing teams, and clients.
• Provide trend analysis and predictive modeling to guide clinical and operational strategies.

Cross-Functional Collaboration
• Partner with Operations, Nurse Advocates, Integrated Care Nurse, and other cross-functional teams to ensure seamless program delivery.
• Facilitate open communication across teams to address challenges, share best practices, and optimize the member experience.
• Contribute to process improvement initiatives that enhance program efficiency and outcomes.

Data Management & Compliance
• Maintain accurate, secure, and compliant documentation of all member and risk pool data in accordance with HIPAA and other regulatory requirements.
• Proactively identify and close data gaps to ensure completeness of risk analyses.
• Uphold the highest standards of confidentiality and data integrity.

Supportive Functions
• Support ImpactAdvocate nurses by coordinating medical records acquisition, conducting provider outreach, and delivering non-clinical member education as appropriate.
• Act as a trusted resource for internal teams, ensuring timely follow-through on requests and escalations.
• Provide initial outreach to overflow members as identified.

    What You’ll Need to Have
    • Bilingual highly preferred
    • LPN licensure in good and active standing is required.
    • At least 1 year of experience in healthcare analytics and risk assessment.
    • Maintain active licensure in good standing in each state where you have members. You may also be required to obtain licensure in a state where you do not have members to enhance team coverage.
    • Ability to flex your schedule to work evening and weekend hours to accommodate members’ schedules.
    • Be able to travel once a year for our corporate gathering, which includes overnight stays.
    • Ability to attend and professionally engage in video meetings with other employees, clients, and members if applicable.
    • Strong technological skills, meaning you can effectively and efficiently use computers, peripheral equipment, and applications/systems, including Microsoft Office products.
    • Autonomous self-starter who is comfortable with ambiguity.
    • Creative mindset and ability to appropriately challenge the status quo.
    • Superb written and oral communication skills.
    • Ability to overcome obstacles with a ‘yes if...’ approach.
    • Ability to effectively balance competing deadlines without losing focus on the bigger picture.
    • Reliable internet and power with a designated area to conduct work with minimal interruptions.
