As part of the multi-disciplinary team with the National University Health System (NUHS)@Home, you will provide quality pharmaceutical care as our Pharmacist.
Job Responsibilities
Review transfer medications appropriately, contact patients pre-transfer on medication supply needed, and answer medicine related questions
Medication counselling on transfer medications by a variety of means (in-person and virtual)
Medication reconciliation
Review discharge prescriptions appropriately
Verify new orders by the physicians and identify inappropriate medication/doses if relevant
Assist with perform medication supply duty
Follow up on medication preparation status
Undertake duties which may be assigned by the NUHS staff member depending on the requirements
Requirements
Bachelor of Science in Pharmacy from an accredited college by Singapore Pharmacy Council
At least 2 years of relevant experience
Fully registered with the Singapore Pharmacy Council
Possess excellent communication, interpersonal and organisation skills
Able to work independently and in a team
Undertake duties which may be assigned by the NUHS staff member depending on the requirement
Weekdays and/or weekends working hours are available
