What is a Virtual Care Physical Therapist Assistant (PTA)? 

A Virtual Care PTA provides critical support to the In-Home Therapy Team at Concierge Home Care. In this role, you will collaborate closely with the interdisciplinary clinical team to deliver virtual physical therapy services to patients through video visits. You will focus on implementing and supporting each patient's established plan of care, as developed by the evaluating physical therapist. Through virtual sessions, you will provide therapeutic interventions, patient education, and ongoing support to help patients progress toward their established functional goals while promoting continuity of care throughout the home health continuum. 

Key Responsibilities

The Virtual Care PTA performs a variety of virtual tasks and assessments to support patient health and reduce hospital readmissions, including:

• Virtual Care & Telehealth Management: Conduct virtual assessments, provide virtual physical therapy in alignment with the patient's plan of care, monitor telehealth data, track patient progress, and facilitate patient care needs across the health continuum.

• Patient & Staff Education: Educate patients and caregivers on relevant medical diagnoses, home safety, and key factors that help lower the risk of hospitalization.

• Psychosocial & Clinical Support: Assess patient psychosocial needs and provides targeted intervention strategies.

• Data Tracking & Trend Analysis: Monitor and track patient data, identify hospital admission trends, and apply preventative care protocols.

• Interdisciplinary Collaboration: Attend and actively participate in scheduled virtual meetings, care conferences, and interdisciplinary discussions related to rehospitalization prevention, patient outcomes, and coordination of care. 

Qualifications

• License: Active, unencumbered Florida Physical Therapist Assistant (PTA) License (Required)

• Certification: CPR Certification (Preferred)

• Technical Skills: Proficiency in general computer navigation, Microsoft Office (including TEAMS and excel) and has the ability to quickly learn and adapt to new technology, including telehealth platforms and EMR documentation systems.

• Remote Workspace & Technology Requirements: The employee must maintain a HIPAA-compliant, private, and distraction-free workspace throughout scheduled working hours and be responsible for maintaining reliable, high-speed internet access to support virtual patient care and clinical responsibilities. 

Schedule:

Full-Time: Monday – Friday
