Job description

Pine Park Health is a value-based primary care practice revolutionizing healthcare for senior living community residents. We empower seniors to get healthy, stay healthy, and lead lives they love through a care model designed with everyone in mind — patients, families, community staff, providers, and payers.

Our approach brings healthcare directly to seniors' homes, offering prevention, screening, chronic condition management, lab work, and diagnostic testing in the comfort of their apartments. With weekly community visits, our teams provide consistent care while collaborating closely with facility staff to address comprehensive health needs. We've eliminated unnecessary barriers to urgent care through same/next-day appointments, helping seniors avoid emergency rooms and hospitalizations where they risk exposure to additional health concerns.

Today, over 185+ communities across Arizona, California, and Nevada trust Pine Park Health, and we're rapidly expanding our reach and impact.

If you're mission-driven and passionate about transforming senior healthcare, this is your opportunity to make a meaningful difference!

The Opportunity

Are you a Nurse Practitioner who wants to build meaningful relationships with patients and practice medicine with the time and support to do it well?

Pine Park Health is hiring a full-time Nurse Practitioner to provide primary care to older adults living in senior living communities across the South Bay. You’ll manage a smaller panel of complex patients, develop personalized care plans, and work closely with RNs, MAs, and care navigators to deliver coordinated, proactive care — with more time to focus on your patients and less time spent navigating the traditional clinic model.

How This Role Works

This is a field-based primary care role serving older adults in senior living communities across San Jose/Santa Clara. You’ll provide care onsite at partner communities, with travel between locations as needed. You’ll have around patient and community needs, with mileage reimbursement for work-related travel. All providers work from home on Fridays, creating dedicated time for documentation, care coordination, and professional development.

Compensation & Benefits

  • Total target cash:$152,000-$195,000 (base + bonus) (Total Comp based on experience and qualification)
  • Equity
  • Work from home Fridays for all providers
  • 6+ weeks of annual PTO, including vacation, sick time, and paid holidays
  • Medical, dental, and vision coverage for you and your dependents
  • 8 weeks of paid parental leave
  • Flexible spending accounts for health and dependent care
  • 401(k)
  • Monthly wellness allowance
  • Annual professional and personal development stipend
  • Company-paid UpToDate subscription
  • Company-issued cell phone
  • Mileage reimbursement

*Final compensation reflects NP experience, training, licensure, and geographic market.

Why NPs Love This Role

  • Smaller panels and real time with each patient
  • RNs and MAs handle coordination, so you can practice at the top of your license
  • No clinic and no waiting room: you go to your patients, on a schedule you help shape
  • Join a team where your input shapes how we deliver care

What You’ll Do

  • Provide comprehensive primary care to older adults in their senior living communities, building ongoing relationships with patients and their care teams
  • Develop and manage individualized care plans that reflect each patient's medical needs, goals, and preferences
  • Deliver monthly Chronic Care Management (CCM) visits and complete the associated documentation
  • Collaborate with RNs, MAs, care navigators, and community staff to coordinate care and support continuity between visits
  • Lead regular clinical huddles to review patient needs, coordinate follow-up, and support effective team-based care
  • Partner with community leadership and care staff to implement care plans in a way that works in each patient's living environment
  • Collaborate with the Medical Director to improve clinical workflows, care standards, and policies, including responses to emerging diseases

What We’re Looking For

Required

  • Active, unrestricted California NP license with Furnishing Number
  • Active, unrestricted DEA registration (Schedules II–V)
  • Interest in caring for medically complex older adults and delivering coordinated, preventive primary care.
  • Willingness to provide onsite care across the South Bay, including driving between partner communities
  • Adaptability and comfort working in a growing, evolving care model.
  • Strong collaborator who values input from every member of the clinical team
  • Clear, effective communication with patients, families, and clinical team members.
  • Comfort with tech-enabled care tools

Preferred

  • Geriatrics experience
  • AGNP-C
  • Value-based care experience
  • Home-based, house-call or mobile practice experience
