Mental Health Specialist – Collaborative Care (CoCM)

About the Role

Carbon Health is seeking a dedicated Mental Health Specialist to join our Collaborative Care Model (CoCM) team. This is a fully remote opportunity for a licensed mental health professional who is passionate about improving access to behavioral healthcare through evidence-based interventions, proactive follow-up, and close collaboration with primary care providers and psychiatric consultants.

As a core member of the Collaborative Care team, you will support patients on an assigned caseload by coordinating their mental and physical healthcare, monitoring clinical progress, and helping ensure they receive appropriate, timely, and effective treatment.

You will work closely with patients' medical providers and the Psychiatric Consultant to assess symptoms, facilitate engagement in care, deliver brief behavioral interventions, and identify patients who may need changes to their treatment plan or more specialized services. Using a patient registry and other clinical systems, you will track outcomes, support treatment adherence, and proactively follow up with patients who are not improving as expected.

This role is ideal for a licensed mental health professional with at least one year of experience in a Collaborative Care setting who enjoys working in a structured, team-based environment and using measurement-based care to improve patient outcomes.

Compensation & Schedule

  • Annual salary: $97,000.
  • Employment type: Full-time.
  • Work arrangement: 100% remote.
  • Location flexibility: Candidates may reside anywhere in the United States, provided they hold an active California license and are willing to obtain a Washington State license.
  • Schedule: Monday through Friday, 8:00 a.m.–5:00 p.m.
  • Hours: 40 hours per week.

Why This Role?

What You'll Do

  • Coordinate care across the primary care team and help patients navigate appropriate behavioral health resources.
Evidence-Based Behavioral Health Interventions
  • Provide brief, evidence-based behavioral interventions tailored to patients' needs and treatment goals.
  • Apply approaches such as behavioral activation, problem-solving treatment, motivational interviewing, and other appropriate brief interventions.
  • Support patients in developing practical strategies to manage symptoms and improve daily functioning.
  • Help patients identify barriers to treatment engagement and work collaboratively to address them.
Treatment Monitoring & Medication Support
  • Systematically track patient symptoms, treatment response, and clinical progress using validated measures and established workflows.
  • Conduct follow-up with patients by telephone or other approved remote methods to monitor symptom changes, treatment adherence, side effects, and potential complications.
  • Support psychotropic medication management by monitoring treatment effectiveness, adherence, and reported side effects, and communicating relevant findings to prescribing medical providers.
  • Identify patients who are not improving as expected and coordinate appropriate follow-up with the care team.
Psychiatric Consultation & Collaborative Treatment Planning
  • Participate in regularly scheduled caseload consultations, typically weekly, with the Psychiatric Consultant.
  • Help prepare and review cases involving patients who are new to the caseload, experiencing limited improvement, or requiring additional clinical attention.
  • Communicate consultation recommendations to the patient's medical provider and support appropriate follow-through.
  • Facilitate treatment plan adjustments in collaboration with the medical provider and Psychiatric Consultant.
  • Help identify patients who may benefit from more intensive treatment or specialized mental health services.
Registry Management & Clinical Documentation
  • Maintain accurate patient tracking and follow-up information in the designated patient registry and other required clinical systems.
  • Document patient contacts, clinical progress, treatment recommendations, and follow-up plans in the EHR and other applicable systems.
  • Use registry data to identify patients who need follow-up, re-engagement, or additional clinical support.
  • Ensure documentation and care recommendations are accessible to the medical provider, Psychiatric Consultant, and other treating professionals as appropriate.
  • Support consistent use of measurement-based care and population-based tracking workflows.
Referrals, Transitions & Relapse Prevention
  • Facilitate referrals to clinically indicated services outside the organization, including specialty mental health care, substance use treatment, social services, housing assistance, and vocational rehabilitation.
  • Coordinate with the care team when a patient's needs exceed the scope of the current treatment plan or available services.
  • Work with patients who have achieved their treatment goals to develop relapse-prevention and self-management plans before discharge from the caseload.
  • Support transitions in care to promote continuity and sustained progress.
