Who We Are

Vigilance Health partners with large healthcare organizations across the country to deliver quality Population Health and Care Management services that move the needle on patient health outcomes. Our care managers work closely and collaboratively with their patients between clinic visits, helping them stay on track with their primary care providers’ treatment and care plans. Our service lines include Chronic Care Management (CCM), Advanced Primary Care Management (APCM), Behavioral Health Integration (BHI), and Remote Patient Monitoring (RPM). We measure success the way our partners do: quality metrics, patient engagement, patient activation, and improved health outcomes that show up in the data.

Our purpose is simple, and we mean it: Change People's Lives — no matter their circumstances.

The Role

The Care Manager is the heart of Vigilance. You'll lead the charge in building meaningful relationships with patients to drive better health outcomes and empower people to take control of their own health. You'll build trust, rapport, and accountability using the principles of Motivational Interviewing and clinical best practices with each patient and with each call.

Above all else, Care Managers are driven by an unyielding passion to make a difference in people's lives. That passion shows up in the work every day — the volume of outreach you put in, the rapport you build, your documentation, and your commitment to the kind of patient engagement, trust, and support that leads to real, long-lasting behavior change.

What You'll Own

Patient Outreach & Engagement

You'll drive a high volume of outbound calls every day — ramping from 5–20 patient connections a day in your first month to 22+ connections by month three — because consistent outreach is what makes everything else possible.

Relationship Building & Rapport

You'll build genuine rapport with your assigned patient panel using Motivational Interviewing techniques, turning first conversations into ongoing relationships patients trust and stay engaged with.

Risk Stratification & Panel Management

You’ll understand the risk level of each patient on your panel and prioritize outreach to keep low-risk patients stable, help higher-risk patients improve, and reduce avoidable ER visits and hospital stays.

Care Coordination & Goal Setting

You'll work collaboratively with your patients to develop SMART goals and patient-centered care plans, with a focus on PCP visits, treatment adherence, care navigation, routine assessments and more — through consistent and engaged outreach.

Documentation & Clinical Best Practices

You'll translate every conversation into clear actions and documentation, applying clinical best practices so the story of each patient's status and progress are captured accurately and can be acted on by the rest of the care team.

What Good Looks Like in 90 Days

  • You're consistently connecting with 22+ patients daily.
  • You've built real rapport with your patient panel and understand their risk stratification.
  • You develop appropriate SMART goals as part of patient-centered care plans, and your work with patients is driving tangible improvements in health outcomes — PCP visits, treatment adherence, baseline data, and care navigation.
  • Your work with patients is producing powerful Patient Success Stories.
