VP, Clinical Operations — Transitions of Care

Build a new post-discharge service line at a growth-stage healthcare services and technology company. You’ll shape the clinical model, scale an existing clinical team, and own outcomes for Medicare Advantage, Medicaid, and Exchange members. This full-time, remote U.S. role reports to the Chief Clinical Officer and offers a $190,000–$220,000 base plus bonus and meaningful equity participation. Specific equity terms will be discussed during the process.

What You’ll Own

• Design the care pathway from admission/discharge notification through the 30-day window: risk stratification, timely in-home/virtual visits, escalation, and follow-up.
• Operationalize medication reconciliation, DME, home health, PCP/specialist scheduling, NEMT, caregiver support, and social-needs interventions into repeatable workflows.
• Build protocols, training, documentation standards, and QA for distributed clinicians; partner on ADT/HIE feeds, workflow tools, and performance data.
• Improve readmissions, PCR and TRC/HEDIS-Stars performance, intervention completion, health-plan ROI, and payer-facing confidence in the model.

What You Bring

• You’ve personally built and launched a post-discharge, transitions-of-care, or readmission-reduction program, then scaled and improved it.
• You know post-acute interventions in practice and can translate health-plan economics, TCM documentation, and quality measures into operations.
• You’ve led distributed clinical teams, ideally in home-based, virtual, payer care management, or post-acute settings.
• You’re a hands-on cross-functional builder; experienced Directors ready for a VP step up are encouraged to apply.
