Field Case Manager – PSPSync Limited
Location: Various Field Locations (Home-Based with Travel)
Reports To: Program Manager
Department: Patient Support Program (PSP) Operations

Position Overview:
The Field Case Manager (FCM) acts as a critical liaison between patients, healthcare providers, and specialty pharmacies, ensuring patients receive timely access to prescribed therapies. Operating in a field-based capacity, the FCM is responsible for providing personalized, high-touch support to patients, educating clinics on PSP processes, and facilitating key program milestones such as reimbursement navigation, access coordination, and adherence support.

Key Responsibilities:
Act as the primary point of contact for assigned clinics, patients, and healthcare professionals within a defined geographic region.

Educate patients and clinic staff on PSP services, medication administration processes, and funding pathways.

Conduct face-to-face and virtual visits with customers to support onboarding, training, and problem resolution.

Facilitate prior authorization submissions, reimbursement navigation, and appeals processes in collaboration with the internal Case Manager team.

Provide adherence support and proactive follow-ups to ensure patients remain on therapy.

Support patients with navigating specialty pharmacy processes and delivery coordination.

Identify barriers to patient access and escalate complex cases requiring further intervention.

Document all interactions and case activities accurately in PSPSync’s database.

Build strong relationships with physicians, nurses, pharmacists, and allied healthcare providers to ensure optimal program execution.

Attend team meetings, training sessions, and client engagements as required.

Provide feedback from the field to enhance program processes and improve service delivery. Assist in other areas as needed from time to time.

Qualifications:
Minimum 3 years of experience in a Case Manager or Field Case Manager role within a Patient Support Program environment.

Background in Nursing (RPN, RN, LPN) or relevant healthcare discipline required.

Solid understanding of reimbursement processes (public/private), prior authorizations, and specialty pharmacy workflows.

Proven ability to manage complex patient cases with empathy, professionalism, and urgency.

Strong interpersonal and communication skills; comfortable engaging with healthcare professionals and patients.

Proficient in using CRM platforms and digital tools for case documentation and communication.

Highly organized, self-motivated, and capable of working independently in a field-based role.

Must possess a valid driver’s license and be willing to travel within the assigned territory.

Compensation: $90,000 - $100,000
