Job description

Prime Home Health is seeking a Bilingual (Eng/Spanish speaking) highly skilled and compassionate Occupational Therapist to join our team on a part-time PRN basis.  As an Occupational Therapist in the Home Health setting, you will play a critical role in helping individuals achieve independence and improve their quality of life in the comfort of their own homes in the northern San Antonio area. Your primary responsibility will be to assess clients' functional abilities, develop personalized treatment plans, and provide therapeutic interventions to address their specific needs. This position offers a unique opportunity to make a positive impact on patients' lives and contribute to their overall well-being.

Responsibilities:

  1. Conduct comprehensive evaluations of patients' physical, cognitive, and psychosocial functioning to determine their occupational therapy needs in a home health setting.
  2. Collaborate with clients, their families, and other healthcare professionals to establish realistic and measurable goals for therapy interventions.
  3. Design and implement individualized treatment plans based on the evaluation findings and in accordance with professional standards and regulatory requirements.
  4. Provide therapeutic interventions and instructions to improve clients' independence in activities of daily living, including but not limited to self-care, mobility, home management, and community integration.
  5. Utilize a variety of evidence-based techniques, adaptive equipment, and assistive devices to maximize clients' functional abilities and promote their overall well-being.
  6. Educate clients, families, and caregivers on therapeutic techniques, safety measures, and strategies to enhance clients' independence and prevent future injuries.
  7. Collaborate with other healthcare professionals, such as physicians, nurses, COTAs and social workers, to ensure holistic care and effective coordination of services.
  8. Maintain accurate and timely documentation of evaluations, treatment plans, progress notes, and discharge summaries as per regulatory and organizational requirements.
  9. Stay updated with advancements in occupational therapy practice, research, and technology through continuous learning and professional development activities.
  10. Adhere to ethical and professional standards of practice, maintaining confidentiality and promoting a positive and respectful environment for clients and colleagues.

Qualifications:

  1. Minimum one year experience  in providing occupational therapy services in an adult home health setting.
  2. Bachelor's or Master's degree in Occupational Therapy from an accredited program.
  3. Bilingual a plus
  4. Texas State License as an Occupational Therapist.
  5. Knowledge of Kinnser/Wellsky EHR and other apps we use for communication, such as Slack. 
  6. Valid CPR BLS certification
  7. Ability to perform comprehensive assessments, develop treatment plans, and deliver evidence-based interventions based on individual clients' needs.
  8. Excellent interpersonal and communication skills to effectively interact with clients, families, and interdisciplinary team members.

Compensation: $75-$80 Per Visit depending on experience
