About the Role:
The Remote Care Nurse II plays a critical role in delivering high-quality nursing care to patients from a remote setting, ensuring their health and wellness are effectively managed through virtual interactions. This position involves comprehensive patient assessments, care planning, and coordination with multidisciplinary teams to optimize patient outcomes while adhering to established clinical guidelines. The nurse will utilize telehealth technologies to monitor patient progress, provide education, and support self-management of chronic conditions, thereby reducing hospital readmissions and improving quality of life. This role requires strong clinical judgment and the ability to respond promptly to patient needs, emergencies, or changes in condition remotely.
