The Nurse Manager for the Medical Management & Transition Team provides operational, clinical, and strategic leadership for Mercy’s virtual-based transitional care medical management and Mercy Lifestyle and Wellness programs. This role is accountable for the daily operations, quality outcomes, staff performance, and regulatory compliance of services supporting patients transitioning across the care continuum, including ED-to-Home- accelerated outpatient management, chronic condition follow-up, and Intense Care Coordination programs.
The Nurse Manager ensures safe, effective, and patient-centered care delivery; fosters interdisciplinary collaboration; ensures timely follow-up and care coordination; and advances Mercy’s mission through high-quality virtual care management and population health strategies.

Position Details:

Qualifications
Education: Graduate of an approved school of nursing. BSN or complete in the next 2 years (already enrolled if possible). 
Licensure: Current licensure by the State Board of Nursing in the applicable state of practice.
Experience: Four (4) years of experience in clinical specialty or related area. Able to apply principles of management, co-worker supervision, teaching, performance improvement, and budget. Basic computer skills including data entry, Windows, and MS Office are required.
Certifications: CPR NRP
Other: To ensure patient safety, it is mandatory that all licensed coworkers who provide direct patient care pass a medication and math competency test before assuming medication administration responsibilities. This exam will cover knowledge of the actions and side effects of commonly used drugs along with math regarding correct dosages and IV problems.
Preferred Education: BS/MS preferred.
Preferred Experience: At least 2 years of supervisory or management experience preferred.
