The Associate Director, Clinical Operations Strategy (Outpatient Rehab) is a critical leadership role responsible for the clinical direction, operational execution, and program advancement if the outpatient rehab service line within the senior living channel. This individual operates as a dyad partner between clinical and operations (designing, implementing, and continuously optimizing the therapy program) across all senior living sites while ensuring alignment with organizational priorities, regulatory requirements, and the senior living operating model.
This role works cross-functionally with operations, analytics, finance, and compliance teams to optimize patient outcomes, improve clinical workflows, and drive scalable models of care with specific accountability to the outpatient rehab program across assisted living, memory care, independent living, and CCRC settings.
This position requires a unique combination of advanced clinical licensure, operational leadership, strategic program development, and the ability to influence cross-functional partners without direct authority over all functions.
Key Responsibilities
- Develop and implement the strategic roadmap for outpatient therapy program across all senior living sites, identifying short- and long-term goals with defined milestones and phased implementation plans.
- Design and standardize therapy program models, clinical workflows, documentation standards and performance frameworks – ensuring consistency and scalability across all sites.
- Own end-to-end site operations from pre-launch, site assessment, equipment procurement approval, through collaborating with local branch clinical and operational leadership for staffing, scheduling, and care delivery.
- Establish evidence-based clinical protocols for the geriatric outpatient population across fall prevention, functional rehabilitation, chronic disease management, and post-acute recovery.
- Monitor and report clinical and operational KPIs including units per clinician, hospitalization avoidance, plan of care goal achievement, and ADC and revenue per community – with regular executive-level reporting to leadership.
- Partners with Strategic Business Development, Sales Leaders, and Senior Living Partnership Executives to ensure program delivery actively supports and protects preferred provider partnerships.
- Lead routine partnership reviews with senior living operators, presenting outcomes data, operational performance, and forward-looking program recommendations that reinforce the preferred provider relationship.
- Serve as a senior-level clinical presence and brand ambassador within the senior living portfolio, ensuring every clinical touchpoint reflects the service excellence standards that differentiate the program competitively.

Required Qualifications
- Advanced clinical licensure required – Physical Therapy (PT) or Occupational Therapist (OT) required; must hold an active, unrestricted license. MBA, MHA or other advanced degrees will be advantageous.
- 5+ years of healthcare management experience and minimum of 5 years of outpatient therapy clinical experience with demonstrated expertise in the geriatric population
- Demonstrated knowledge of Medicare Part B billing, medical necessity documentation, and compliance requirements.
- Excellent oral and written communications skills, including the polish, poise, and executive presence that will ensure effective interaction with executive level audiences and key stakeholders internally and externally.
- Current or recent experience in a complex, matrixed company, with proven ability to influence leaders and key stakeholders in such an environment.
- Highly collaborative mindset and excellent relationship-building skills, including the ability to engage diverse stakeholders and win their co-ownership in the outcome.
- High strategic agility with strong problem solving and critical thinking skills, using data to inform decision-making.

In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.
Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$115,200 - $158,400 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
