Job Title: Healthcare/Patient Outreach Certified Medical Assistant

 

Schedule Hours: Monday – Friday 8:00am - 5:00pm

 

Department: HealthSync

 

Location: 17401 Commerce Park Blvd Tampa FL

 

**After 90 days, position will become eligible to work remote**

 

**MUST HAVE 2-3 YEARS EXPERIENCE AS A CERTIFIED MEDICAL ASSISTANT**

 

***Remote position is stationary - NOT able to travel with remote equiptment***

 

Position Summary

We are seeking an experienced and patient-centered Patient Outreach Medical Assistant to support our Primary Care teams by connecting with FMCOH patients recently discharged from the hospital. This role plays a critical part in ensuring safe transitions of care, preventing readmissions, and strengthening continuity with our Primary Care physicians.

 

You will review discharge information, reconcile medications, and schedule timely follow-up appointments across 16 Primary Care offices. This position is ideal for an MA who enjoys patient interaction, is confident reviewing medications, and thrives in a structured outreach/call‑center–style workflow.

Responsibilities:

Essential Functions

  • Conduct outbound calls to FMCOH patients recently discharged from the hospital.
  • Review discharge instructions and perform medication review/reconciliation with the patient.
  • Educate patients on the importance of timely follow-up with their Primary Care provider.
  • Schedule follow-up appointments across multiple FMCOH Primary Care offices.
  • Track call volume, outreach attempts, and patient outcomes in the EMR.
  • Communicate with Primary Care offices regarding patient needs, scheduling barriers, or clinical concerns.
  • Maintain a warm, supportive, and professional phone presence throughout all interactions.
  • Follow established workflows to ensure consistent, high-quality transitional care

  • Comfortable with high-volume outbound calling and patient engagement.
  • Confident reviewing medications and discussing discharge instructions with patients.
  • Strong communication skills — clear, friendly, and patient-focused.
  • Organized and able to manage outreach lists across multiple offices.
  • Enjoys working independently while staying connected to a larger care team.
  • Professional, compassionate, and committed to improving patient outcomes.
Qualifications:

Education/Training

  • High school graduate or equivalent.
  • Successful completion of Medical Assistant training from an approved technical school (or at least one (1) year of verifiable and applicable work experience as a Certified Medical Assistant in a patient care setting) required.

Licensure/Certification

  • Maintains current BLS Healthcare Provider Certification – and
  • Maintains current certification or registration by one of the following agencies:
  • Certified Clinical Medical Assistant (CCMA) by National Health career Association (NHA) (www.nhanow.com)
  • Certified Medical Assistant (CMA) by the American Association of Medical Assistants (AAMA) (www.aama-ntl.org)
  • Clinical Medical Assistant Certification (CMAC) by the American Medical Certification Association (AMCA) (www.amcaexams.com)
  • National Certified Medical Assistant (NCMA) by National Center for Competency Testing (NCCT) (www.ncctinc.com)
  • Nationally Registered Certified Medical Assistant (NRCMA) or Nationally Registered Certified Advanced Medical Assistant (NRCAMA) by the National Association for Health Professionals (NAHP) (www.nahpusa.com)
  • Registered Medical Assistant (RMA) by American Medical Technologists (AMT) (www.amt1.com) , the American Registry of Medical Assistants (ARMA) (www.arma-cert.org), or by American Allied Health (AAH) (www.americanalliedhealth.com)
  • Applicants with an accepted Medical Assistant certification or registration from an agency not listed will be considered on a case-by-case basis.
