POSITION PURPOSE

Ensures the coordination of the next level of service/care/needs for the patient with incidental findings and lung cancer screening on radiology exams through the daily tracking board.  Acts as a resource to patient, family or physician calling for assistance, and works collaboratively with the FIND/LCS program Nurse Navigators.  

ESSENTIAL FUNCTIONS

1.   Knows, understands, incorporates and demonstrates the mission, vision and values of Trinity Health in leadership, behaviors, practices and decisions.

2.   Tracks and documents interventions and outcomes. Ensures appropriate data are collected to track program interventions and outcomes. Ensures appropriate and timely communication of patient progress to referring physicians and other care providers

3.   Coordinates scheduling/referring of patient to specialty providers throughout the healthcare system. Prepares patients for appointments, providing information in a clear understandable manner and providing written follow-up in the manner patient prefers (text, email, mail etc.) Serves as a direct contact and resource for patient and family, providing emotional support and education.  

4.   Acts as a resource for patient scheduling, clinical resource management, patient access and others as necessary to ensure appropriate patient access and follow-up. Demonstrates excellent critical thinking skills as they interview patients and gather information to assure accurate and timely assessment and intake into the program, entering information into all computer systems necessary.  

5.   Reviews radiology reports. Compiles and reviews information contained within reports to assist and support the following of algorithms, generation of documents, and formalized processes for transition in commonly followed care pathways.  

6.   Attends care conferences, tumor boards and other meetings as necessary. Ensures that appropriate patient data are available for conference.  

7.   Collaborates with program stakeholders in support of data collection for registry participation, quality reporting, etc.   

8.   Collaborates on performance improvement activities as indicated by outcomes in program efficiency, patient experience and outcomes.  

9.   Provides feedback instrumental to current program change to support ongoing improvement of the program model. 10. Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution.

11. Maintains the confidentiality of information acquired pertaining to patient, physicians, colleagues, and visitors to Trinity Health. Discusses patient and hospital information only among appropriate personnel in appropriately private places.

12. Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.

13. Performs other duties as assigned.

14. Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health Integrity and Compliance Program and Code of Conduct, as well as other policies and procedures to ensure adherence in a manner that reflects honest, ethical, and professional behavior.

MINIMUM QUALIFICATIONS

1.   High school diploma.  At least one year of post-high school medical education.  Associate degree preferred.  

2.   At least one (1) year of clinical oncology health care or hospital-based experience.  

3.   Demonstrated interpersonal skills in order to interact and communicate with patients and medical staff.

4.   Comprehensive understanding of medical terminology.

5.   Ability to function autonomously, self-directed with a high level of judgment and decision-making.

6.   Demonstrated organizational skills necessary to develop and manage multiple monitoring systems.

7.   Demonstrated proficiency in computer databases and software to include Excel, Word. Prefer prior Epic experience.

8.   Demonstrated ability to effectively problem solve with internal and external customers and systems.

9.   Must be comfortable operating in a collaborative environment.

 10. Must possess a personal presence characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Health.

Pay Range- $21.95- $32.92
