As a Population Health Navigator I, you play a key role in supporting population health initiatives by coordinating care and resources in collaboration with Primary Care Providers and the healthcare team. Using a comprehensive, holistic, proactive and “Patients First” approach, you will help patients and their care partners navigate the Cleveland Clinic Health System, schedule appointments, connect to community resources and address health-related needs. In addition, you will assist with care coordination and process improvement within the Ambulatory Care Management Department to enhance patient outcomes, caregiver experience and overall care efficiency. This role directly supports Cleveland Clinic’s mission and vision by advancing innovative care models for senior patients in Medicare Advantage plans, fostering collaboration, continuous learning and a culture of excellence dedicated to improving care for specific patient populations.

A caregiver in this role works remotely from 8:00 a.m. – 1:00 p.m.

Candidates must reside in the Independence area and be available to attend in person for equipment pickup and any required on-site meetings.

A caregiver who excels in this role will:

  • Build relationships with an assigned patient population within the primary care practice to ensure optimal health and wellness while collaborating across the continuum to assist in patient navigation for high, moderate and rising risk patients.

  • Assist with coordinating population health activities to help improve quality and safety outcomes for an assigned patient population.

  • Perform activities to proactively close care gaps, assist with health maintenance, schedule office and preventative care visits and assist with navigating health insurance benefits and coverage via telephone, letter, or MyChart.

  • Assist with PCP fidelity.

  • Pend orders for PCP and schedule follow up appointments.

  • Manage in basket messages and incoming calls from patients and caregivers.

  • Perform onboarding and off-boarding activities to assist patients with seamless provider-related transitions.

  • Perform data gap closure in the payor portals.

  • Collaborate effectively with all members of the navigation and primary care teams, leadership and multi-disciplinary caregivers.

  • Support and participate in process improvement efforts.

  • Provide coverage to peers.

  • Assist with Cleveland Clinic Community Care, Value Based Operations and Ambulatory Care Management initiatives.

  • Complete duties with minimal direct supervision.

  • Adhere to policies and procedures to control and mitigate risks associated with job function.

Minimum qualifications for the ideal future caregiver include:  

  • High School Diploma or GED and two years of recent experience working with patients in a clinic setting, or other equivalent position, which includes interacting with patients, physicians and other departments in a collaborative manner

  • OR an Associate’s or Bachelor’s degree and one year of experience

  • Ability to adapt to changing technology, processes, policies and procedures while effectively managing priorities and time

  • Strong organizational, communication (verbal and written) and relationship-building skills

  • Proficient in data collection and analysis and Microsoft Office

  • Experience delivering exceptional customer service and resolving conflicts professionally

  • Working knowledge of HIPAA regulations

Preferred qualifications for the ideal future caregiver include: 

  • Associate’s or Bachelor’s Degree

  • Knowledge of medical terminology

  • PSS (Patient Service Specialist) and MA (Medical Assistant) experience

Physical Requirements:

  • Must be able to sit for prolonged periods of time.

  • Possess good visual acuity through normal or corrected vision.

  • Possess dexterity to operate a PC and other office equipment.

  • Light driving to outside locations within the region.

Personal Protective Equipment:

  • Follow standard precautions using personal protective equipment as required.
