Your expertise:
- Conduct thorough and multi-disciplinary assessments of patients, ensuring compliance with standard operating policies and procedures.
- Utilize your clinical knowledge and expertise to accurately determine patients' eligibility for Continuing Healthcare services.
- Collaborate with the CHC team and relevant stakeholders to ensure the effective planning and management of patients' care needs.
- Provide expert clinical advice and support to team members, patients, and senior leads, ensuring the delivery of high-quality care.
- Act as a Duty Clinician when required, addressing urgent cases and ensuring timely responses.
- Maintain accurate and up-to-date data systems, complying with the Data Protection Act 1998 and upholding confidentiality and data protection principles.
- Participate in ongoing professional development and clinical supervision, adhering to NMC guidelines and maintaining your professional registration.
- Support and contribute to the implementation of improvement programs, enhancing the quality and efficiency of CHC services.
- Collaborate with care providers to identify and arrange services that meet the needs of patients qualifying for NHS Continuing Healthcare.
- Conduct investigations into patients' eligibility for NHS Continuing Healthcare following disputes or challenges.
- Champion the personalization agenda within CHC, ensuring that services are delivered to maximize patient safety and choice.
- Work collaboratively with internal and external stakeholders, fostering constructive relationships to achieve shared goals.
- Ensure accurate and professional communication with patients, representatives, and colleagues.
- Support the implementation and evaluation of projects, providing high-quality information and analysis to facilitate decision-making.
- Provide oversight and leadership for audit and quality improvement initiatives within the CHC team.
- Prepare high-quality reports and presentations for meetings, analysing performance data and outcomes of cases.
- Demonstrate adaptability, flexibility, and a positive attitude in a busy and changing environment.
- Engage in ongoing research and development to stay informed about national best practices and advancements in CHC.
- Act in accordance with relevant legislation, organizational policies, and codes of conduct, ensuring the highest ethical standards.
