Schedule: Full-Time
Pay Rate: $16.00 - $17.00 per hour

Overview

Chronic Care Staffing is seeking a highly organized and detail-oriented Scheduling Coordinator to support our patient referral and care coordination processes. In this role, you'll serve as a vital connection between patients, healthcare providers, and external specialists, ensuring referral documentation is processed accurately and efficiently.

Responsibilities

  • Review electronic health records (EHRs) to gather and verify patient information.
  • Retrieve and organize essential documentation, including:
    • Demographic sheets
    • Insurance cards
    • Recent office visit notes
  • Prepare and submit referral packets to external medical specialists.
  • Process referrals through fax, phone, and secure healthcare portals.
  • Utilize internal specialty resource guides to identify appropriate referral destinations.
  • Ensure referral packages are complete, accurate, and compliant with company standards.
  • Collaborate closely with follow-up and care coordination teams.
  • Cross-train on additional workflows to support operational needs.
  • Monitor documentation quality and identify missing or incorrect information before submission.

Requirements

Must Have

  • 1–2 years of experience in:
    • Medical office administration
    • Healthcare clerical support
    • Health administration
    • Patient services coordination
  • Comfortable navigating Electronic Health Records (EHR) systems.
  • Strong computer and data entry skills.
  • Exceptional attention to detail.
  • Ability to manage multiple tasks and prioritize workload effectively.
  • Strong written and verbal communication skills.
  • Ability to work independently in a remote environment.

Nice to Have

  • Experience handling patient referrals or authorizations.
  • Familiarity with specialty care terminology such as Dermatology, ENT, Cardiology, Orthopedics, etc.
  • Previous experience working with healthcare scheduling platforms.
  • Understanding of insurance verification processes.
