Job description

About Leap

Leap is one of the fastest-growing benefits solutions and a category-defining pioneer in employer specialty pharmacy. We are reshaping how life-changing therapies are delivered and financed, ensuring patients get the treatment they need while employers finally get a fair deal.

Specialty drugs and infusions represent nearly 10% of all healthcare spend and are the fastest-growing cost category for employers. Leap tackles this challenge with a novel approach: eliminating hidden markups, expanding access to high-quality infusion providers, and bringing clarity and fairness to how therapies are priced and paid for.

We’re proud to partner with numerous Fortune 500 companies and leading TPAs. Each patient we serve creates immediate ROI: lower costs, improved access, and better care. Join us as we redefine what’s possible in specialty care.

About the Role

The Care Guide plays a pivotal role in ensuring each patient's treatment journey is smooth, informed, and empowering. Acting as the central point of contact, you will coordinate logistics, provide medication education, and liaise between patients, providers, and nurse staffing agencies. This role is ideal for a compassionate healthcare professional who thrives in a fast-paced, patient-first environment.

Key Responsibilities

  • Serve as the primary contact for patients, responding to inquiries with clarity, empathy, and efficiency.

  • Educate patients and providers on home infusion processes, including medication handling, storage, and administration protocols.

  • Schedule and confirm treatment appointments, coordinating with nursing agencies to ensure timely, accurate care delivery.

  • Verify medication receipt and comprehension with patients prior to treatment.

  • Ensure clinical documentation is shared with providers post-infusion and that billing information is properly routed to the finance team.

  • Collaborate cross-functionally with the Customer Success team to maintain strong client relationships and support service excellence.

Qualifications

Required:

  • 3–5 years of nursing or pharmacy experience, preferably with infusion therapy or specialty medications.

  • Active clinical license or certification: Registered Nurse (RN), Certified Pharmacy Technician (CPhT), or Medical Assistant (MA).

  • Strong patient coordination or case management background.

  • Excellent written and verbal communication skills.

  • High level of organization and attention to detail; able to juggle multiple responsibilities seamlessly.

  • Comfortable working remotely and independently.

  • Available to work full-time Pacific Standard Time (PST) hours.

  • A strong patient-first mindset, with a focus on improving care experiences and outcomes.

Preferred:

  • Experience in a healthcare startup or fast-paced care delivery environment.

  • Fluency in Spanish (spoken and written)
