Our client, a Research, Care and Health Financing company, is looking for a Care Coach for their Remote location.
Responsibilities:
Develop a relationship with members and establish trust as a source to help ease friction in managing their health.
Assist members in creating a healthy vision and goals for their overall health & wellbeing.
Develop goals and care plans in collaboration with members, interprofessional team and health care providers.
Improve member’s health literacy by providing education and support as defined by client clinical pathways.
Appropriately adapt digital coaching and education methods to accommodate the unique physical, psychosocial, cultural, age - specific needs of each client served.
Reinforce SMART goals and utilize behavioral techniques in development of plans with members.
Provide an empathetic environment.
Identify and help to eliminate barriers to following the care plan i.e., costs, economic constraints.
Demonstrate an ability to deliver effective digital and telephonic coaching in a manner that promotes behavior change.
Work in close collaboration with physicians, certified diabetes educators and other healthcare professionals in understanding of significant social and environmental factors underlying members’ health.
Collaborate continuously with an interprofessional team in care delivery, with a focus on improving measures and outcomes.
Manage risk by initiating escalation to specialists based on client clinical criteria.
This includes, but is not limited to cases requiring deeper clinical assessment or education, medication review and optimization, or any other timely clinical care.
Implement creative approaches for outreach and care services for members who are hard to engage or are lost to follow up.
Demonstrate an understanding of member needs and deploy essential virtual and community resources as needed to maintain engagement and optimize care
Demonstrate established customer service behaviors and standards.
Requirements:

Moderate level of sophistication using web apps, able to use information dense productivity tools

Strong analytical and problem solving skills
Strong relationship-building and problem-solving skills
Experience with the use of behavioral techniques such as goal-setting, problem-solving and/or motivational interviewing
Experience in direct education and/or coaching with individuals for disease prevention, health promotion and behavior change
Certified in a nationally recognized coaching program
Knowledge of disease prevention and basic understanding of exercise principles, nutrition and weight management
Direct experience with people living with diabetes, prediabetes, hypertension, cardiac disease or depression.
