Job description

Flagler Health is building the clinical operating system for modern musculoskeletal care.

We partner with MSK provider groups and specialty clinics to help them grow, operate more efficiently, and deliver better longitudinal care across patient acquisition, clinical workflows, and ongoing patient engagement. Our platform sits at the intersection of care delivery and clinic operations, helping providers capture more value across the full patient lifecycle.

We’ve recently raised our Series B and are entering our next phase of growth. We’re hiring a Head of Marketing to build and scale the marketing engine that will drive that growth.

This is a pipeline and revenue role. Success means generating high-quality pipeline from the right provider groups, and helping convert that into revenue in a complex, trust-driven market.

We are hiring a Vice President of Clinical Operations to lead the strategic oversight and day-to-day management of Flagler Health's clinical operations infrastructure, and to take the function from early traction to a durable, repeatable operating engine.

Reporting to the Chief Medical Officer and Chief Executive Officer, this leader will own the scaling of our RTM, CCM, and PCM programs while partnering closely with Product, Engineering, and clinical leadership to drive cross-functional performance. This is not a "sit back and manage" role. As VP of Clinical Operations, you will operate as a player-coach—leading from the front on critical programs while developing and scaling a distributed clinical workforce.

You will own program-level operating targets, standardize clinical workflows, and evolve the operating model to support larger patient populations, new service verticals, and continued multi-program expansion. This is a high-impact role at a critical inflection point, with the opportunity to shape how Flagler scales its clinical organization.

Location: Based in New York City.

Key Responsibilities

Clinical Program Leadership: Lead and scale RTM, CCM, and PCM clinical operations across all patient populations; define program standards, protocols, and quality benchmarks to ensure consistent, high-quality care delivery.

Workforce Management: Recruit, manage, and develop a distributed team of 50+ clinicians and care staff; oversee performance evaluations, credentialing compliance, and ongoing training to maintain clinical standards and workforce readiness.

KPI-Driven Operating Cadence: Establish and maintain a data-driven operational cadence including capacity planning, utilization tracking, and workflow optimization; develop and monitor KPIs to evaluate program health and identify areas for improvement.

Cross-Functional Collaboration: Partner closely with Product and Engineering to design, standardize, and continuously improve clinical workflows; translate operational requirements into actionable specifications and ensure seamless integration of technology platforms.

Operational Controls & Scalability: Develop and implement operational controls, policies, and procedures that strengthen infrastructure and support rapid growth; identify and resolve bottlenecks that limit clinical throughput or patient access.

Strategic Business Development: Lead the expansion of new service verticals, including remote therapeutic services; design go-to-market operational models that increase revenue and improve patient outcomes.

Data Analysis & Reporting: Apply business analytics methodologies—including statistical analysis, regression modeling, and data-driven planning—to evaluate program performance and present actionable insights to senior leadership.

Regulatory & Compliance Oversight: Ensure operations comply with applicable federal and state regulations and billing requirements; maintain documentation and audit readiness across all programs.

Patient Experience: Oversee the end-to-end patient journey across all clinical programs, from onboarding through ongoing care management, ensuring a high-quality, consistent experience for 6,000+ active patients.

Vendor & Technology Management: Manage relationships with third-party technology platforms and service vendors; evaluate and integrate tools to enhance operational efficiency and scalability.

Executive Reporting: Provide regular performance updates, strategic roadmaps, and operational reports to the CMO, CEO, and Board; contribute to investor materials by articulating operational milestones and growth metrics.

Requirements

  • 8–12+ years of experience in operations management, clinical operations, or program leadership, with meaningful time in a leadership or player-coach role within high-growth environments.

  • Proven track record of designing and implementing scalable operational processes, standardizing workflows, and building performance management systems in a high-growth or startup setting.

  • Experience scaling clinical or care-delivery operations beyond early stage, including managing distributed teams and driving consistent performance across multiple programs.

  • Strong understanding of healthcare workflows, value-based or technology-enabled care models, and multi-stakeholder operating environments. Familiarity with RTM, CCM, or PCM programs is strongly preferred.

  • Strong foundation in business analytics, including KPI development, capacity planning, statistical analysis, and data-driven decision making.

  • Demonstrated ability to operate across both strategic and tactical levels—from standardizing workflows to building the systems and processes behind them.

  • Experience with change management, process design, and rolling out operational programs across large or distributed teams.

  • Strong vendor and technology management skills, with experience evaluating and integrating platforms to improve efficiency at scale.

  • Exceptional written and verbal communication skills; ability to present complex operational and analytical findings to senior leadership, the Board, and external stakeholders.

  • Comfort working cross-functionally with Product, Engineering, and clinical leadership teams.

  • Bachelor's degree required; MBA or equivalent advanced degree, ideally with a concentration in business analytics, strongly preferred.

Non-Functional Skills & Cultural Fit

  • Credible, steady leader who can inspire distributed clinical teams while holding a high performance and quality bar.

  • Thrives in fast-paced, high-growth environments and is comfortable operating through ambiguity.

  • Highly execution-oriented with a willingness to roll up sleeves and stay close to the work.

  • Resilient, adaptable, and outcomes-driven, with a strong sense of ownership.

  • Balances strategic thinking with operational discipline and clinical rigor.
