Roles & Responsibilities

  • Fully licensed by the NC governing board regulating Human Services professions, or a licensed RN in NC with four years MH/DD/SA experience
  • Master's degree in a human services field with one year MH/DD/SA experience or Bachelor's degree in a human services field with two years MH/DD/SA experience, or a substance abuse professional with equivalent supervised experience
  • Minimum two years LTSS/HCBS coordination, care delivery monitoring, and care management experience if serving LTSS needs (can be concurrent with above requirements)
  • Valid driver's license and residence within Trillium’s catchment area

Requirements:

  • Provide complex care coordination and monitoring for members with IDD (may include secondary MH/SA or complex medical needs) through multidisciplinary collaboration and referrals, including TCM opt-in/opt-out, care needs screenings, 90-day transition plans, CMCA updates, and care plan development
  • Primary focus on Autism Spectrum Disorder, particularly children ages 18 months to 15 years, using person-centered planning to gather information and address individual needs
  • Conduct Discovery activities and NC Medicaid 1915i assessments; inform members and legally responsible persons about available Medicaid/state-funded services, options, processes, and planning
  • Ensure timely service coordination and documentation; provide education and support to members and legally responsible persons about rights and responsibilities; complete planning activities and maintain up-to-date care plans

What We’re Looking For


Trillium Health Resources has a career opening for an RCT (RB-BHT) Care Manager to join our team! The RCT (RB-BHT) Care Manager is responsible for providing complex care coordination and monitoring to members having a primary Intellectual or Developmental Disability, which may include a secondary mental health or substance abuse, and/or complex medical/physical health need. The RCT (RB-BHT) Care Manager assists with integrated care management through multi-disciplinary collaborations and referrals, including but not limited to: assistance with the TCM opt in/opt out process, completion of care needs screenings, 90 day transition plans, completion/updates of CMCAs and care plan development, and service coordination as needed/deemed appropriate. This position has a primary focus on members that have been diagnosed with Autism Spectrum Disorder, particularly children (18mos – 15yo). 


On a typical day, you might:


  • Utilize person centered planning methods/strategies to gather information, to get to know the members supported and to adequately address specific needs of the individual.
  • Provide education and support to members in learning about and exercising rights and responsibilities.
  • Scheduling and completion of NC Medicaid 1915i Assessment.
  • Complete Discovery activities (information gathering and assessment) as a component of the planning process.
  • Ensure that members/legally responsible persons are informed of services available (all types of Medicaid and state-funded services), service options available (e.g. Individual/Family Direction for Innovations participants), processes (e.g. requirements for specific service), Natural and community services, and so forth.

Qualifications


Required:

  • Must meet Qualified Professional (QP) status. 
  1. Fully licensed by the NC governing board regulating Human Services professions; OR Fully licensed RN who is licensed to practice in the state of NC by the NC Board of Nursing who also has four (4) years of full-time experience with the MH/DD/SA population served. OR 
  2. A graduate of a college or university with a master’s degree in a human service field and has one year of full-time, pre or post-graduate degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has one year of full-time, pre or postgraduate degree accumulated supervised experience in alcoholism and drug abuse counseling; OR 
  3. A graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has two years of full-time, pre or post bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; OR 
  4. A graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has four years of full-time, pre or post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling. 
  • Additionally, If serving members with Long Term Services and Support (LTSS) needs, the care manager, must meet the minimum QP requirements defined above (A-D) and shall additionally have at a minimum two (2) years of prior LTSS and/or Home and Community Based Service (HCBS) coordination, care delivery monitoring and care management experience. This experience may be concurrent with the years of experience required to become a Qualified Professional.
  • Must have a valid driver’s license.
  • Must reside within Trillium’s catchment area, which includes the following counties: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, or Wilson.
  • Must be able to travel within catchment as required.
