Roles & Responsibilities

Current license as Physical Therapist or ability to obtain in the state of practice
Previous experience in outpatient setting (outpatient experience) preferred
New grads welcomed
Schedule availability: Monday to Friday, no evenings or weekends

Requirements:

Provide physical therapy services to residents in on-site clinic or in their rooms, within the facility (no travel to homes outside the building)
Develop and implement individualized treatment plans, monitor progress, and adjust interventions as needed
Market the clinic, manage your own scheduling, and work independently
Collaborate with interdisciplinary teams to deliver patient-centered care across Assisted Living, Independent Living, and Outpatient settings

Physical Therapist – Outpatient

We welcome Consistent weekly PRN, part time and remote interest

For the remote, you will need to treat in person on the 6th visit

Location: Urbana Place Senior Living, Brooklyn Park, MN

Setting: Assisted Living, Independent Living, Outpatient,

Lovely Community - Work with higher functioning residents

Schedule: Monday to Friday, No Weekends, No Evenings
