Position Summary:
The Director, Operations Delivery Lead collaborates with Healthmap Medical Directors, Product, Clinical Services Leadership, and Operations to develop and execute population health strategies to enhance our Kidney Health Management (KHM) program and its multi-faceted provider engagement model, including, but not limited to, engagement with Primary Care Physicians, Nephrologists, Cardiologists, Endocrinologists, Acute Care Facilities, Dialysis Centers, and our internal case management programs.  This role requires strong operational expertise, clinical acumen related to chronic kidney disease and its various associated comorbidities (e.g., cardiovascular disease), and a value-based care orientation focused on improving clinical and quality outcomes across our patient population ultimately resulting in reduced cost of care.
Responsibilities:
  • Leverage strong understanding of Healthmap population health and provider engagement model for chronic condition management of kidney disease to position our program to deliver targeted engagement, clinical, and financial outcomes across our patient population
  • Ensure our program consistently delivers a strong value proposition to our provider partners, spanning Primary Care, Specialty Care, and Healthcare Facilities
  • Enable our team of clinical/non-clinical provider-facing staff through clinical program development, technology and data enablement, and enterprise learning management
  • Drive improved clinical and quality outcomes and reduced cost of care by promoting industry-accepted and evidence-based guidelines, value-based care principles, and established standards of care for a kidney population
  • Leverage data & analytic insights to provide perspective on current program performance and opportunities for enhancement, leading program design and implementation to realize improved outcomes for our patient population
  • Strong cross-functional collaboration with various departments across Healthmap, including Medical Directors, Product, Clinical Services Leadership, Operations, Business Intelligence, Clinical Quality, and Enterprise Learning Management
  • Perform other duties as assigned
 
Requirements:
  • Bachelor’s degree in healthcare administration, business administration, or a related field (Master's degree preferred). Equivalent experience and applicable certifications may be considered in lieu of education requirement.
  • Minimum of 10 years of progressive clinical leadership experience in a healthcare related field
  • Nursing degree or clinical licensure such as Social Worker, Registered Dietitian, Pharmacy Tech, preferred, but not required
  • Strong business acumen with a proven track record in executing strategic initiatives, project management, problem solving, and delivering operational commitments
  • Exceptional interpersonal and communication skills, with the ability to influence and collaborate with diverse stakeholders
  • Ability to analyze complex data, identify trends and implement evidence-based solutions
