Roles & Responsibilities

Bachelor's degree in social work, psychology, or related field
Experience in disaster case management or community/social services
Valid driver's license and ability to travel to client locations as needed
Strong interpersonal, communication, and coordination skills with multiple service providers

Requirements:

Conduct Needs Assessments for disaster-impacted individuals and families
Develop and monitor Service Plans with clients to ensure timely completion
Facilitate appropriate community referrals and link clients to additional services
Maintain ongoing contact with clients to ensure quality services and progress toward goals

Job description

JOB PURPOSE:

The DCM Case Manager will provide disaster case management services to individuals and families impacted by disasters. The Case Manager is responsible for completing Needs Assessments, developing Service Plans and making appropriate community referrals for additional services. The Case Manager maintains appropriate contact with the client and ensure quality services are provided and that their Service Plan is completed timely.
