Work Schedule & Location:
This position is fully remote. The required schedule is 20 hours per week, typically structured as three days per week from 11:00 AM–4:00 PM, with remaining hours considered flexible to accommodate additional appointments and tasks outside of scheduled core hours. Occasional in-person attendance may be required for team collaboration or meetings (e.g., monthly or quarterly), as determined by program needs.
Position Summary:
The Behavioral Health Care Manager (BHCM) is a licensed professional who serves as a key member of the Collaborative Care Team (CoCM) between BTST Services and the partnered Pediatric Facility. This role bridges pediatric medical care with behavioral health services, providing direct support to children, adolescents, and families. The BHCM works in partnership with primary care providers (PCPs) and psychiatric consultants to coordinate care, deliver brief interventions, and support patients through all phases of behavioral health treatment.
Key Responsibilities:
Manages a caseload of pediatric patients with behavioral health needs
Ensures timely access to care, continuity of services, and responsiveness to patient needs
Conducts standardized behavioral health screenings
Delivers brief, evidence-based interventions
Educates patients and caregivers on diagnoses, treatment options, and care plans
Tracks patient progress using a clinical registry
Collaborates with primary care providers and psychiatric consultants to update and adjust care plans
Monitors symptoms, medication side effects, and treatment outcomes using validated tools
Participates in weekly psychiatric consultation meetings
Documents all patient interactions and care activities in the Electronic Health Record (EHR)
Facilitates referrals to additional services when needed
Supports discharge planning, including relapse prevention and self-management strategies
Other duties as assigned
Reporting Structure:
The BHCM reports to the Director of Community Support Programs and serves as a key member of the Integrated Behavioral Health Team. In this role, the BHCM is expected to:
Actively participates in interdisciplinary team meetings
Participates in psychiatric case reviews
Engages in continuous quality improvement initiatives
Maintains timely, accurate, and complete clinical documentation
Meets established quality metrics related to screening, follow-up, and symptom improvement
Utilizes a collaborative, family-centered approach to care delivery
Participates in ongoing training and supervision for professional development
Ensures delivery of high-quality behavioral health services
