FLSA Status: Non-Exempt

Posting Salary Range: $54,106 - $67,210

Office Location: Remote within Trillium’s Catchment area (See requirements section for included counties)

What We’re Looking For


Trillium Health Resources has a career opening for a Neighborhood Connections Coordinator to join our team! The Trillium Neighborhood Connections Coordinator role has been created for defined health communities to take part in grassroots solutions for Community Development. The Trillium Neighborhood Connections Coordinator will take part in efforts to address housing related challenges, transportation, food insecurity, employment and education challenges, interpersonal safety and toxic stress, as well as community inclusion and isolation. This position will assist members in connecting to organizations and programs in the community that assist with addressing unmet health related resource needs.


On a typical day, you might:


  • Receive and distribute referrals for members related to unmet health related resource needs.
  • Receive escalated SDOH referrals. 
  • Remain knowledgeable regarding community-based organizations and their resources. Compile and update information as needed in designated platforms. 
  • Represent Trillium Health Resources through involvement in regional, community programs and inter-agency conferences and meetings. May be required to make presentations to the Trillium Board or other groups. 
  • Cultivate and maintain effective relationships with agency personnel, county and state officials, community organizations/human service organizations and the general public.
  • Work collaboratively with members, in-house care team, and providers to link them with relevant resources to address unmet health related resource needs using a person-centered approach.
  • Provide dedicated support for internal Trillium teams and external providers and community-based organizations.

Qualifications


Required:

  • Must meet QP status. 
  1. Fully licensed by the NC governing board regulating Human Services professions; OR Fully licensed RN who is licensed to practice in the state of NC by the NC Board of Nursing who also has four (4) years of full-time experience with the MH/DD/SA population served. OR 
  2. A graduate of a college or university with a Master’s degree in a human service field and has one year of full-time, pre or post-graduate degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has one year of full-time, pre or postgraduate degree accumulated supervised experience in alcoholism and drug abuse counseling; OR
  3. A graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has two years of full-time, pre or post bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; OR
  4. A graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has four years of full-time, pre or post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling. 
  • Must have a valid driver’s license
  • Must reside within Trillium’s Catchment of North Carolina. Counties included: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, Wilson.
  • Must be able to travel within catchment as required.


Preferred:

  • Lived experience, Community Development experience, and/or experience with community-based organizations and/or community partners centered around housing, food insecurity, transportation, education & employment, interpersonal violence/toxic stress and/or social integration and inclusion.
