Roles & Responsibilities

  • Registered Nurse or Allied Health Professional (Occupational Therapist, Physiotherapist, Dietitian, Exercise Physiologist, Podiatrist or Psychologist) with relevant registration.
  • Experience in care coordination and delivering seamless and effective support services to people with complex and chronic care needs.
  • Experience in primary health care and care planning, with strong clinical knowledge and ability to translate care plans into practice.
  • Desirable: previous experience delivering care virtually and digitally, and understanding health behaviour change methodology and how to apply it in practice.

Requirements:

  • Deliver care coordination and health coaching via video, telephonic, and digital programs to support people with complex and chronic care needs in the community.
  • Promote health literacy and empower participants to self-manage their health through a coordinated and integrated approach.
  • Facilitate health behaviour change and care coordination by translating care plans into practical actions and collaborating with the care team.
  • Support remote, flexible service delivery within Amplar Preventative Health Team, coordinating schedules within operating hours (08:00 AM – 06:00 PM AEST, Monday to Friday).

The Opportunity  

 

The Care Coordinator is part of our amazing Amplar Preventative Health Team. In this role you will work through a variety of service delivery modes –video and telephonic delivery or digital programs to support people with complex and chronic care needs in the community.

 

Drawing on your holistic knowledge and background in health, you will support health literacy and empower participants to self-manage their health. You will utilise a coordinated and integrated approach, facilitating health behaviour change and care coordination in the delivery of health coaching.

 

This is a unique permanent full or part-time (minimum 0.6FTE) and work from home opportunity, so you can be located anywhere in Australia. It is required to have suitable office area from which to work and reliable internet connection. 

 

Our service operates between the hours of 08:00AM - 6:00PM (AEST) Monday to Friday (availability to work Friday is highly desirable.)

 

 

About You 

 

You are an advocate for preventative health and have the natural ability to build rapport with a wide range of clients and healthcare professionals. You have experience working in care coordination and delivery of support services to people with complex and chronic care needs.

 

You are Registered Nurse or Allied health Professional (Occupational Therapist, Physiotherapist, Dietician, Exercise Physiologist, Podiatrist and Psychologist) with relevant registration.

 

In addition, you will have the following experience:

  • Care coordination and the delivery of seamless and effective support services to people with complex and chronic care needs.
  • Experience in primary health care and care planning.
  • Strong clinical knowledge and the ability to translate care plans into practice.
  • Strong understanding of chronic disease management practices, including workflows across the care team.
  • It is desirable but not essential that you have:
    • Previous experience delivering care virtually and digitally, and
    • An understanding health behaviour change methodology and how this can be applied in practice.
