Outpatient Intake Clinician Canaan, CT (Hybrid Remote)

About the Position:


The Intake Clinician is responsible for collaborating with a multi-disciplinary treatment team to create an individualized recovery experience that incorporates the body, mind, and spirit of each client.  The Intake Clinician will conduct comprehensive intake assessments for clients seeking outpatient services. This role involves gathering detailed client histories, identifying presenting issues, and formulating initial diagnoses to inform individualized treatment plans. The clinician will work closely with clients to understand their needs and provide appropriate referrals and recommendations for treatment.


Schedule:


Tuesday: 12:00PM - 8:30PM

Wednesday: 12:00PM - 8:30PM

Thursday: 8:00AM - 4:30PM

Friday: 8:00AM - 4:30PM

Saturday: 8:00AM- 4:30PM


Your Role:

  • Perform detailed intake assessments to determine the appropriate level of care for clients.
  • Gather comprehensive client histories, including substance use and mental health backgrounds.
  • Provide accurate and clinically-driven assessments for substance use disorders and mental health conditions.
  • Utilize standardized screening and assessment tools to identify client needs.
  • Facilitate referrals for urinalyses of admitting clients.
  • Make informed recommendations based on assessment findings.
  • Coordinate and facilitate referrals for appropriate services and treatment programs.
  • Create comprehensive and collaborative treatment plans, including referrals for Family Wellness, Psychiatric Services, MATS, etc.
  • Coordinate client treatment with the client, applicable OPS program, and staff.
  • Collaborate with family members, former providers, and referents to ensure comprehensive care.

Qualifications:

  • Graduate Degree in Mental Health Counseling, Marriage and Family Therapy, Social Work, Psychology, or related field, required.
  • Holds and maintains licensure as: LMSW/LCSW, LMFT, LMHC, or LPC in CT, NY, NJ, and/or MA, required.
  • Willingness to apply and obtain licensure in all the following states in which one is not yet licensed; CT, NY, NJ, MA, required.
  • Previous experience/knowledge in substance abuse and/or co-occurring disorders integrated treatment, required.
  • Graduation from a CACREP accredited program or NASW Social Work accredited program, preferred.
  • Previous experience working in an outpatient setting, preferred.

Compensation:


The base rate of pay for this position is $65,000 - $80,000 per year. Actual pay is determined based on a number of job-related factors including skills, education, training, credentials, experience, scope and complexity of role responsibilities, geographic location, performance, and working conditions.
