Posting Salary Range: $66,240 - $88,595

What We’re Looking For


Trillium Health Resources has a career opening for a State Hospital Liaison to join our team! The State Hospital Liaison is responsible for providing complex care coordination targeting those with chronic, unresolved, or complex physical, behavioral health and social determinant needs. This includes providing care planning with foundations in national evidenced based and informed standards to do whole person care. The State Hospital Liaison completes required documentation/paperwork/tasks in a software platform according to timelines. This position will serve as a member of the State Hospital integrated care team for the State Psychiatric inpatient setting. The State Hospital Liaison is expected to assess and respond to members’ health and safety when the Trillium Catchment is under a state of emergency or weather advisory.


On a typical day, you might:


  • Engage members in care coordination/care management process with reflective listening and motivation interviewing to assess the mental health, medical and social needs of the member to successfully transition to independent living within a timely and effective manner. 
  • Provide education about all available services, natural and community supports, treatment options, diagnosis, etc.
  • Linkage to needed psychological, behavioral, educational, physical, and dental evaluations. Identifies, links or if appropriate. 
  • Facilitate referrals for health-related services, community-based resources and social support services, and community reintegration supports.
  • Ensure the coordination of care with each member’s primary care physician and/or other identified specialist/dental provider.
  • Complete CMCA and any other assessments needed or provides linkage to professional qualified to complete evidenced based assessments.

Qualifications


Required:

  • Master’s degree and full licensure by the State of North Carolina as a LCSW, LPA, or LCMHC and minimum of one (1) year of relevant experience working directly with individuals with a serious mental illness (SMI); OR
  • Bachelor’s level Registered Nurse (RN) licensed in North Carolina and a minimum of one (1) year of relevant experience working directly with individuals with a serious mental illness (SMI).
  • The following immunizations are required for this position: 
  • Fully vaccinated against Covid-19, Influenza (annually within Influenza season Nov 1 – March 31), MMR, Varicella and Tdap (Tetanus, diphtheria pertussis). 
  • MMR and Varicella - must show proof of vaccination or have blood drawn to show immunity. 
  • Tdap - must show proof of one dose. 
  • Employees must provide proof of all immunizations prior to their start date or agree to provide proof of all immunizations within 4 months of start date.
  • Fully licensed by the state of North Carolina as an LCSW, LPA, LCMHC, or bachelor’s level RN licensed in North Carolina is required.
  • Must have a valid driver’s license
  • Must reside in North Carolina and within 75 miles of Wayne County.
  • Must be able to travel within catchment as required.


Preferred:

  • Master’s Degree in Human Services field.
