Roles & Responsibilities

  • Licensed and board-certified Nurse Practitioner in Texas (or eligible) with a current TX state license
  • Experience delivering care via telehealth, including virtual visits, telephonic/video assessments, and remote chronic disease management
  • Strong communication skills, clinical judgment, and ability to collaborate with physicians, RNs, and interdisciplinary teams
  • Knowledge of state regulations, clinical protocols, and collaborative practice agreements; commitment to value-based care

Requirements:

  • Provide adult and geriatric patient-centered clinical care through telehealth visits, telephonic and video assessments
  • Develop and manage medical plans, remote chronic disease management, risk assessments, and remote triage
  • Collaborate with physicians, RNs, case managers, and other team members to diagnose, manage, and prevent acute and chronic conditions
  • Ensure compliance with state regulations, clinical protocols, and cooperative practice agreements while delivering high-quality remote care

Job description

Location: Remote (Telehealth), serving Members across Texas

Travel Required: Minimal; occasional in‑person meetings or trainings as needed

Schedule: Full‑time role providing virtual visits, telephonic/video assessments, and remote administrative responsibilities

Reports to: VP Advanced Primary Care Practice

Job Summary

The Telehealth Nurse Practitioner (NP) provides holistic, compassionate, and patient‑centered care to adult and geriatric Members through virtual visits, telephonic/video assessments, and remote chronic disease management. This full‑time role focuses on delivering high‑quality clinical care in a remote environment while collaborating with physicians, RNs, case managers, and interdisciplinary teams to diagnose, manage, and prevent acute and chronic conditions.

 

The NP is responsible for Plan Medical Management, virtual health risk assessments, chronic disease management, remote triage, and care coordination. This position requires strong communication skills, clinical judgment, and the ability to deliver value‑based care through telehealth platforms while maintaining compliance with state regulations, clinical protocols, and collaborative practice agreements.
