Roles & Responsibilities

  • Graduate of an accredited Physician Assistant program or accredited school of advanced nursing (Nurse Practitioner)
  • Current license to practice as a Physician Assistant or Nurse Practitioner; ability to obtain and maintain any required national certification (e.g., NCCPA for PA) and state licensure
  • Effective communication and collaboration skills; self-motivated with the ability to work with physicians, case management, patients, and families
  • Clinical experience in a private practice or institutional setting; demonstrated commitment to ongoing education and professional development (e.g., maintenance of BLS/ACLS and relevant certifications)

Requirements:

  • Conduct medical histories and physical examinations and provide care within the medical plan of care; collaborate with physicians, case management, patients and families; initiate advance care planning as appropriate
  • Order, interpret, and manage labs and imaging studies; perform or assist with routine procedures within scope; develop and implement treatment plans and participate in end-of-life decision-making with hospice teams when indicated
  • Coordinate discharge planning and transitions of care, including admission/discharge medication reconciliation and completion of discharge summaries
  • Maintain professional requirements and accurate documentation, including ongoing CME, licensure certification, and proficiency with the Epic EMR system in accordance with hospital policies and procedures

Job description

Physician Assistant: Completes medical histories and physical exams and provides treatment to HMSS patients and consultative services to appropriate specialty groups in accordance with the medical plan of care; critical pathways established by Self Regional Healthcare, Self-Medical Group, and Medical/Nursing Policies /Procedures /Protocols and Standards of Care. Completes admission / discharge medication reconciliation, orders and interprets labs and imagining studies, and completes discharge summaries. Works in collaboration with Physicians, Case Management, patients and families. Initiates advance care planning; works with supportive care and hospice teams to assist patient and family with end-of-life decisions. Performs any other assigned duties per HMSS Physician, VP Of Medical Affairs, and Self Medical Group Administration. Maintains continuing medical education requirement in accordance with hospital requirements; maintains BLS and ACLS certifications. Minimum 1-year clinical Physician Assistant experience required.

Graduate of an accredited PA program, certified by the National Commission on Certification of Physician Assistants, NCCPA National Certification Exam completion and passing, current licensure, clinical expertise, leadership, and organizational ability.  Must be able to communicate effectively and work collaboratively with others. Must be self-motivated. Must have significant practice experience in private practice or institutional setting.

Nurse Practitioner: Completes medical histories and physical examinations to appropriately provide care to patients in accordance with the medical plan of care and within own scope of practice. Uses a systematic approach in assessing the patient’s physical, psychological, and social status to determine the need for care, the type of care to be provided, and the need for reassessment/follow-up.   Demonstrates ability to perform and/or assist physician(s) and other healthcare providers with routine procedures within scope of practice and expertise. Delivers care in a non-judgmental/non-discriminatory manner that is sensitive to the linguistic, cultural, and spiritual needs of the patient, family, and other caretakers. Uses Epic electronic medical record system proficiently as primary source of documentation of all patient visits and correspondence. Completes all required training when scheduled allowing none to become delinquent. Completes all requirements to maintain current licensure at all times, ensuring no suspension or lapse occurs. Practices work in a safe manner, following proper protocols of infections control, accident prevention, proper body mechanics, etc.  Attends meetings/training sessions as directed. Performs other duties as assigned.

Graduate of an accredited school of advanced nursing. Must be currently licensed to practice as an advanced nurse practitioner by the State Board of Nursing for South Carolina.
