Job description

HomeCare Homebase (HCHB) EMR Integration Application Specialist 

Are you passionate about technology and making a real impact in patient care? The Care Team is seeking an Integration Application Specialist to join our growing team. In this dynamic role, you'll serve as a key liaison between clinical operations and IT, helping implement and optimize the hospice software system, Homecare Homebase (HCHB). This position is fully remote, and our regular operating hours are Monday through Friday, 8:30 am to 4:30 pm EST. 

You'll work cross-functionally to support application integrations, manage system updates, troubleshoot issues, and ensure end users are trained and equipped to succeed. This position plays a critical role in driving efficiency, improving data quality, and enhancing our overall delivery of care through the use of smart technology.
