Roles & Responsibilities

  • Bachelor’s degree in social work, clinical counseling, public health, community development or related field, OR two years’ refugee service provision or service coordination experience

Requirements:

  • Provide ongoing, in-person case management: screening interviews, needs assessment, development of individualized wellness plans with SMART goals, and monitoring progress and outcomes
  • Deliver direct patient care by meeting participants face-to-face (weekly to quarterly) in clinic, community, or via home visits; coordinate and attend appointments with participants across services
  • Coordinate connections to services within Saint Alphonsus and external partners across medical, mental health, case management, massage/acupuncture, community garden, legal services
  • Conduct community outreach and disseminate program information to stakeholders; collect and report program data to leadership

The Center for Global Health and Healing is hiring for a team-oriented and compassionate Service Coordinator for the Program for Survivors of Torture, located in Boise, near the Regional Medical Center!

*This is a grant funded position. The current grant is through October 2027, with the possibility of being renewed. This position does quality for all benefits.

Position Summary & Highlights:

  • As the Service Coordinator for the Programs for Survivors of Torture (PSOT), you will provide service coordination to eligible participants and their families under the PSOT Program.

  • The PSOT program works under The Center for Global Health & Healing as an expanded service to the refugee community since 2015.

  • For survivors and their families, PSOT offers interdisciplinary and integrative services to help address the physical, psychological, social, and spiritual impact that torture and trauma has on the person.

  • ***This position will work in-person at the International clinic or in the community with the clients. This position does not work remotely or hybrid.***

What Will You Do:

  • You will be responsible for ongoing, in-person individual case management including:

    • Conducting a screening interview with potential clients

    • Facilitating the participant’s needs assessment

    • Developing an individualized wellness plan based on SMART goals and identified needs

    • Monitor services in accordance with the wellness plan

    • Monitoring the plan for positive outcomes

  • This is a direct patient care position. You will meet with participants either weekly, or at least quarterly face-to-face - in the clinic, in the community, or by home visit; this is a large part of the position.

  • This may include connecting eligible participants with services within the Saint Alphonsus Health System and outside community partners.

  • This may include attending appointments with participants in the community, as they pertain to a focus area of the program:

    • Medical services

    • Mental health services

    • Case management

    • Service coordination

    • Massage/Acupuncture

    • Community Garden

    • Legal services

  • You will also be responsible for providing community outreach and information on the program to local stakeholders and for collection of program data and providing that information to program leadership.

An ideal candidate will have the following experience:

  • Thorough understanding of the current refugee process as it relates to resettlement, healthcare, mental health and broader social services needs. As well as various other immigration processes.

  • An in depth understanding, or willingness to learn, of trauma informed, culturally responsive and linguistically appropriate care.

  • Experience in case management, community outreach, and working in a diverse multidisciplinary team.

Work Schedule:

  • 40 hours/week, either 4 - 10 hour shifts (flexible on Mon. - Thurs. or Tues. - Fri.) or 5 - 8 hour shifts (most work 8:00am - 4:30pm, but flexible)

Minimum Requirements:

  • Must possess knowledge normally acquired through completion of a Bachelor’s degree in social work, clinical counseling, public health, community development or related field, OR two years’ experience (volunteer or paid) in a refugee service provision context and/or experience providing service coordination required.
