Hospice Clinical RN Liaison/General In-Patient

Roles & Responsibilities

  • Registered Nurse with a valid Connecticut license or eligible to obtain one.
  • Nursing diploma or Associate degree in Nursing required; Bachelor's degree in nursing preferred.
  • Minimum 3 years of nursing experience in an acute care setting with at least 1 year in hospice; 1 year Home Care and Hospice GIP experience preferred.
  • CPR certification required (must be maintained); valid driver's license, car insurance, and access to a dependable vehicle to travel for multiple stops; ability to obtain CPR within 90 days if not currently certified.

Requirements:

  • Collaborate with Administrator, Clinical Manager, and Manager of Referral Coordination to address agency and patient needs; spend significant time in YNHHS hospitals coordinating care and transitions.
  • Receive patient referrals electronically or in hard copy and perform on-site evaluations for Home Health and Hospice eligibility, coordinating with care providers and discharge teams to establish care plans.
  • Perform on-site General Inpatient (GIP) evaluations, admissions, and hospice GIP visits at assigned hospitals as needed, and track patient readmissions.
  • Educate clinicians and groups in the clinical community about Yale New Haven Health at Home programs; participate in staff meetings, required trainings, and special projects (e.g., remote patient monitoring, Advanced Care at Home).

Under the direction of the Administrator and Manager of Clinical Operations, the clinical liaison represents Yale -New Haven Health System/Home Care Plus in the clinical community by initiating and maintaining relationships with referral sources.

Responsibilities:
  • Works with Administrator, Clinical Manager, and Manager of Referral Coordination to focus on agency and
    patient needs.
  • Spends considerable time in YNHHS hospitals working directly with patients, care providers, and Care
    Management on care coordination and transition planning.
  • Receive patient referrals electronically or via hard copy.
  • Onsite evaluations of patients who may be eligible for Home Health and Hospice, coordinate with care providers, care management, and discharge teams in acute and rehab facilities to establish a care plan and/or safe transition to care.
  • Perform on-site general inpatient (GIP) evaluations, admissions, and hospice GIP visits at assigned hospitals
    as needed.
  • Tracks patients readmitted to hospitals.
  • May deliver orders for physician signature.
  • Meets with individuals and groups in the clinical community to educate regarding programs and services offered by Yale New Haven Health - Health at Home and Yale New Haven Health - Health at Home Southeast.
  • Attends staff meetings, in-services, and all other required meetings.
  • Compliance with all mandatory requirements inclusive of HealthStream.
  • Assists with specialty projects and pilots such as remote patient monitoring, GIP, Advanced Care at Homeand other as indicated.
  • May perform other duties as assigned.
Qualifications:

EDUCATION

 

Nursing diploma or an Associate degree in Nursing required. Bachelor's degree in nursing preferred.

 

EXPERIENCE

 

Minimum of three (3) years nursing experience in an acute care setting with at least one (1) year in Hospicerequired. Minimum of one (1) year Home Care and Hospice GIP experience preferred.

 

LICENSURE

 

Registered Nurse with a valid (or eligible) Connecticut license required.
A valid driver's license and proof of car insurance are required when using a personal vehicle for company business .
Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within three (3) months (90 days) of hire.

 

SPECIAL SKILLS

 

Excellent verbal communication, customer service and written skills.
Knowledge of home care and hospice eligibility desired.
Must have self-motivation and excellent time management skills.
Ability to effectively build relationships.
Excellent computer skills.

 

PHYSICAL DEMAND

 

Periods of sitting and standing.
