  • Care coordination with physicians and other healthcare entities, including patient referrals, MD office visit notes, and sharing of Plan of Care with other entities
  • Tracking of supplemental physician orders and plans of care
  • Input of basic non-clinical information into the EMR and other related duties as assigned

Responsibilities:

What You Will Do:

 

· All tasks as directed by RN Case Managers, Nurse Manager, or Area Clinical Leader.

· Care coordination with physicians and other healthcare entities. This includes, but is not limited to:

  • Patient referrals
  • MD office visit notes
  • H&P
  • Sharing of Plan of Care with other entities
  • Tracking of supplemental physician orders and plans of care
  • Input of basic non-clinical information into the EMR
  • Performs other related duties as assigned
Qualifications:

What You Will Bring:

  • Ability to exercise independent judgment
  • Ability to demonstrate above accuracy attention to detail to ensure accuracy
  • Ability to read and interpret medical record data for chart audit

Education and Experience:

  • High school diploma or GED required
  • CMA/QMA preferred
  • One (1) year experience in care coordination preferred
  • Knowledge of service regulations and process
  • Phone/computer experience
