Schedule: Hybrid, (Monday - Friday) 8:30 am-5:00 pm, Evenings and weekends, as needed

Are you passionate about providing specialized clinical case management services to diverse populations? We are seeking a dedicated individual to join us in this impactful role. As a Clinical Case Manager, you will provide a broad range of services within the guidelines established by the Organization and specific programs. You will work with a high degree of independent accountability, adhering to professional and ethical standards, organizational policies, and contractual requirements. The populations you will serve may include children at risk of human trafficking, adults who have experienced trafficking, families, immigrants, refugees, and individuals with mental illness, HIV/AIDS, and/or substance abuse issues. Your responsibilities will include connecting participants to healthcare, behavioral health, and social services such as housing, adult education, and employment assistance programs. Your individual accountabilities and work volume will be established through the development of annual Success Objectives, providing a clear framework for your professional growth and impact.

Work Arrangement:
The role is a “hybrid” or “remote” arrangement, primarily involving home-based work and community engagement to meet participants, it is expected this person would be able to meet for “in person” meetings when needed or at the request of others. In-person meetings may be required on a regular basis (weekly, monthly, etc.). There may also be the availability to work from home when appropriate.
