Roles & Responsibilities

  • Graduation from an accredited LVN nursing program with current valid LVN license
  • Three years of experience in public health nursing, acute care, case management, and/or in-home health care, including a minimum of two years in managed care case management focused on inpatient and/or outpatient ambulatory care
  • Experience in, and willingness to be part of, a multidisciplinary team and ability to coordinate care with social work, ambulatory case management, hospitalists, and physicians
  • Minimum BLS certification or higher per American Heart Association or American Red Cross guidelines

Requirements:

  • Provide daily care coordination with concurrent review for patients in hospital or SNF via telephonic communication and deliver case management for admitted patients and those enrolled in ambulatory case management
  • Identify California Children Services (CCS) cases, manage transfers, and perform retro reviews
  • Work as part of an interdisciplinary care team, coordinating care with social work, ambulatory case management, Regional Medical Directors, hospitalists, hospital nursing staff, and clinic physicians
  • Ensure smooth transitions between inpatient and ambulatory settings and advocate for patient needs in inpatient and ambulatory care contexts

Job Overview

The LVN Case Manager will provide daily care coordination with concurrent review for patients in the hospital or Skilled Nursing Facilities (SNF) telephonically. This position will also provide case management to patients who are admitted to the hospital or SNF, and those patients who may need to be enrolled in ambulatory case management. The LVN case manager will be responsible for identifying (California Children Services) CCS cases, handling transfers, and retro reviews.  Works as part of an interdisciplinary care team coordinating care and collaborating with social work, ambulatory case management, Regional Medical Directors, Hospitalists, the hospital nursing personnel, and the physician in the clinic.

Minimum Requirements

  • Graduation from an accredited LVN nursing program.

  • Current valid License as a Licensed Vocational Nurse

  • Experience in and willingness to be part of a multi-disciplinary team.

  • Three years’ experience in public health nursing, acute care, case management, and/or in-home health care required; minimum of 2 years of managed care experience in case management with a focus on inpatient and/or outpatient ambulatory care required.

  • Bilingual in English and Spanish preferred.

  • A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Compensation

$32.54 - $40.67 hourly
