  • Provide education and training to clinicians and leaders on coding accuracy and documentation best practices; develop tools and resources; serve as subject matter expert in coding, auditing, and compliance

The Opportunity:

The Home Health and Hospice Coder & Auditor is responsible for reviewing, coding, and auditing clinical documentation to ensure accuracy, compliance, and optimal reimbursement. This role combines expert knowledge of ICD-10 coding guidelines, OASIS data set requirements, and CMS regulations to support quality patient care and organizational compliance. The Coder & Auditor works closely with clinicians, compliance, and leadership teams to ensure documentation integrity, accurate coding, and identification of training opportunities.

Key Responsibilities:

  • Coding & Documentation Review
    • Assign accurate and compliant ICD-10-CM codes for home health and hospice episodes based on medical documentation and OASIS assessments.
    • Ensure coding aligns with CMS guidelines, payer requirements, and industry best practices.
    • Review clinical documentation for completeness, accuracy, and demonstration of medical necessity and appropriate levels of care.
    • Review clinician documentation for completeness, accuracy, and medical necessity.
    • Collaborate with clinicians to clarify diagnoses, treatments, and documentation gaps.
  • Auditing & Compliance
    • Conduct internal audits of clinical records for Home Health, Hospice, and Palliative Care programs to ensure compliance with regulatory and organizational standards.
    • Monitor trends in coding and documentation to identify potential compliance issues.
    • Prepare audit reports and recommend corrective actions.
    • Assist in the development of compliance policies and procedures related to coding and documentation.

Quality & Performance Improvement (QAPI):

  • Actively participate in QAPI initiatives, contributing data and analysis from coding and documentation audits to support performance improvement plans.
  • Provide insight into coding and documentation trends impacting clinical quality outcomes, star ratings, and value-based purchasing metrics.
  • Collaborate with the Quality and Clinical Leadership teams to develop action plans for identified areas of improvement.
  • Support the collection, validation, and reporting of quality indicators (OASIS, HIS, CAHPS, etc.).

Education & Training:

  • Provide individualized and group education to clinicians and leaders on coding accuracy, documentation best practices, and regulatory compliance.
  • Develop tools and resources to promote accurate and compliant documentation for Home Health, Hospice, and Palliative Care.
  • Serve as a subject matter expert and resource for coding, auditing, and compliance questions.

Service Fee:

Industry standards applicable to the state depending on work experience and level of expertise.

Hours:

US Eastern Standard Time (EST) – 9am to 6pm
