Telos Health Systems is a rapidly scaling, multi-state healthcare platform delivering integrated clinical services across skilled nursing facilities and senior living communities. The Chief Operating Officer (COO) is responsible for building, scaling, and optimizing the operational infrastructure that enables Telos Health Systems to deliver consistent, high-quality clinical services across a growing national footprint.
This role requires a leader of exceptional operational discipline and personal drive. This executive will integrate people, processes, and technology into a highly efficient operating platform capable of supporting rapid geographic expansion and service-line growth. The COO will translate strategic vision into disciplined execution, ensuring operational excellence, clinical consistency, and strong financial performance.
The COO partners in close collaboration with the CEO, Chief Medical Officer, Chief People Officer, Business Development, Finance, and other teams to support growth and ensure seamless onboarding of new partners, effective deployment of clinical teams, and reproducible outcomes across markets.
This role requires a leader who combines deep, hands-on execution skills with strategic thinking and has a proven ability to build systems that scale in complex healthcare environments.
• Operationalize Telos’ growth strategy across geographies and service lines
• Build a high-accountability, metrics-driven operating system and organization
• Ensure consistent, high-quality clinical delivery across all markets
• Drive performance in value-based care arrangements
• Establish scalable infrastructure for multi-state expansion and integration
• Design and implement operating systems, processes, and governance structures for multi-state growth
• Develop enterprise operational playbooks across onboarding, provider deployment, credentialing, and facility engagement
• Define and track KPIs across productivity, utilization, and service delivery
• Partner with clinical and finance leadership to align operations with risk-based contract performance
• Drive continuous operational improvement
• Oversee operational infrastructure supporting clinical teams
• Optimize provider staffing, scheduling, and productivity
• Build scalable onboarding, credentialing, and training systems
• Support implementation of care models improving post-acute outcomes
• Drive urgency and follow-through on provider contracting and deployment — ensuring speed of execution
• Lead onboarding of new partner facilities and health systems
• Standardize client onboarding, ramp, and performance monitoring
• Ensure consistent, high-quality partner experience
• Support expansion with new and existing partners
• Partner with CEO and other C-Suite executives to manage performance against financial targets
• Improve productivity, scheduling efficiency, and service economics
• Optimize revenue cycle operations including documentation and charge capture
• Drive margin improvement through operational discipline
• Support deployment and optimization of EMR, RCM, scheduling, and analytics systems
• Enable data-driven decision-making across operations
• Identify automation and workflow optimization opportunities
• Work with C-suite teams to ensure compliance with CMS, HIPAA, and state regulations
• Support quality initiatives and value-based care performance
• Implement monitoring and audit frameworks
• Build and lead a high-performing operations organization
• Develop regional and functional leaders

• 12–15+ years of healthcare operations leadership experience
• Experience in multi-site healthcare organizations
• Post-acute, SNF, or senior living experience strongly preferred

• Deep understanding of clinical workflows and provider models
• Experience managing P&L drivers and operational economics
• Strong leadership and organizational development capabilities
