Care Partnerships Associate - Remote - CST/PST preferred - Full-time

The Care Partnerships Associate will play a key role in building and expanding strategic partnerships that drive patient referrals and long-term partner satisfaction. This is a high-impact, outbound-facing role ideal for someone who brings a sales or business development mindset — with the follow-through to back it up.
You’ll be responsible for growing partner relationships through proactive outreach, creative problem-solving, and a deep understanding of what makes healthcare partnerships succeed. This role is perfect for someone who’s entrepreneurial, metrics-driven, and thrives in environments where you create the playbook.
What You’ll Be Doing
Own and grow a portfolio of healthcare partnerships — with a clear focus on driving referrals and increasing partner engagement
Proactively reach out to providers, referral coordinators, and practice managers to strengthen relationships and spot growth opportunities
Identify patterns in referral behavior and recommend tactics to improve performance
Execute outreach campaigns (calls, emails, in-person if applicable) to build awareness and drive partner retention
Collaborate cross-functionally with marketing, clinical operations, and analytics to support partner needs and campaign execution
Create monthly reports, dashboards, and presentations that track performance and growth metrics
Support pre-launch and launch strategies for new markets, ensuring all stakeholders are aligned
Maintain CRM and internal tools (Salesforce, Google Drive, Monday.com) with up-to-date, actionable insights
Always be looking for ways to add value and streamline the partner experience
Actively participate in internal meetings and training, sharing ideas and best practices
What You Bring to the Table
2–5 years of professional experience; business development, outreach, or sales experience highly preferred
Strong communication and relationship-building skills - You’re comfortable on the phone, in person, and via email
High level of ownership - You don’t wait for direction: you initiate, follow up, and follow through
Problem-solving mindset - You’re able to troubleshoot partner issues with creativity and urgency
Comfortable with data - You know how to look at performance metrics and translate them into actions
Organizational skills with the ability to juggle multiple priorities in a fast-paced environment
Experience with CRM tools (Salesforce preferred) and productivity platforms (Google Suite, Monday.com)
Bachelor’s degree (Healthcare, Marketing, Communications, Business, or related field)
