  • Associate’s degree in Occupational Therapy with four years of documented experience working with co-occurring physical health and behavioral health conditions, physical health, or chronic conditions, or an equivalent education/experience
  • Fully licensed by the North Carolina Board of Occupational Therapy as an Occupational Therapy Assistant
  • Valid driver’s license
  • Must reside within Trillium’s Northern or North Central Region in North Carolina (counties: Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, Warren, Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrell, Washington, Wilson)

Requirements:

  • Screen Trillium members with identified behavioral health, physical health, co-occurring, co-morbid, or multi-morbid conditions for occupational therapy needs affecting transitions from the most restrictive to least restrictive environments
  • Participate in multi-disciplinary care teams to contribute to integrated care
  • Monitor the care plan, service delivery, and health and safety of the member
  • Evaluate home environments for hazards or barriers to independent living

FLSA Status: Exempt

Posting Salary Range: $54,434 - $70,122

Office Location: Remote in North Carolina within Trillium’s Northern or North Central Region.

What We’re Looking For

 

Trillium Health Resources has a career opening for a Complex Transitional Care Occupational Therapy Assistant (OTA). The OTA is responsible for providing input for Complex Care Coordination, targeting those with chronic, unresolved or complex physical, behavioral health and social determinant needs. This includes providing care planning with foundations in national evidenced-based and informed standards to provide whole person care. The COTA completes required documentation/paperwork/tasks in the Trillium software platforms according to established timelines.

 

On a typical day, you might:

 

  • Screen Trillium members who have identified needs with behavioral health, physical health, co-occurring, co-morbid, or multi-morbid conditions for conditions/issues within the area of occupational therapy that impact transitions from most restrictive environments to least restrictive environments.
  • Participate in multi-disciplinary care teams to contribute to integrated care.
  • Monitor the care plan, service delivery, and health and safety of the member.
  • Complete occupational therapy screening tools as needed.
  • Evaluate home environments for hazards or barriers to independent living.

Qualifications:

 

Required:

  • Associate’s degree in Occupational Therapy and four (4) years documented experience working with the co-occurring of physical health and behavioral health, physical health, or chronic condition populations; OR
  • Equivalent combination of education/experience.
  • Fully licensed by the North Carolina Board of Occupational Therapy as an Occupational Therapy Assistant.
  • Must have a valid driver’s license.
  • Must reside within Trillium’s Northern or North Central Region in North Carolina. Counties included are Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, Warren, Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrell, Washington and Wilson.
  • Must be able to travel to a Trillium office location and within catchment as required.

 

Preferred:

  • Experience in all 3 areas above.
