Description

The ConcertoCare Social Worker provides clinical care to medically and socially complex patients, who live in community-based settings including assisted living facilities, adult family homes, and private homes. This position ensures that patient’s biopsychosocial needs are adequately met through completing thorough behavioral health assessment and engaging the patient, caregiver(s), and care team in treatment planning and care coordination activities. Social Workers are responsible for the provision of brief therapies and crisis interventions as a behavioral health expert using evidenced-based practices aligned with an integrated model of healthcare. Social Workers work collaboratively with other members of the local care teams and are key in ensuring access to equitable behavioral health care in alignment with the organization’s mission, vision, and values.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Supports the behavioral health needs of patients on an assigned patient caseload in accordance with the established clinical practices of the behavioral health team
  • Uses evidence-based models of care to deliver brief treatment for common mental health issues such as depression, anxiety, coping with loss, mood management, and substance-use disorders
  • In some settings, provides consultation to group residences (such as assisted living facilities) to ensure facility staff are able to recognize changes in patients behavioral health functioning and appropriate interventions to support the needs of our patients
  • Support psychotropic medication management by collaborating with prescribing providers, supporting treatment adherence needs, partnering with patients/caregivers for continuous monitoring of efficacy and potential negative side effects, and coordinating with internal psychiatrist and clinical care teams to address urgent psychotropic issues
  • Participate in regularly scheduled caseload reviews and clinical team meetings; facilitate effective communication regarding patients behavioral health needs across all members of the clinical team both verbally and in writing
  • Complete and document assessment of patients in a timely manner
  • Facilitate treatment plan changes for patients who are not demonstrating improvement in symptoms by consulting with patient’s primary care provider/members of the clinical team and using measurement-based care to attune behavioral health interventions to the existing needs and symptom burden of patients
  • Demonstrate the ability to serve as a subject matter expert in a particular area(s) of behavioral health care and supports the behavioral health team at large by sharing knowledge and resources related to area of expertise
  • Ensure care is delivered in a manner that honors patient’s diverse values, beliefs, behaviors, backgrounds, social, cultural, and linguistic needs
  • Document patient encounters, referral activities, and other pertinent information in electronic health record ensuring all encounter notes are locked within 72 hours of the encounter date

QUALIFICATIONS

  • Master’s degree in Social Work.
  • Current Rhode Island state LICSW or eligible for such within 60 days of employment.
  • 2+ years of prior healthcare/behavioral health experience required.
  • Strong problem-solving and organizational skills
  • Geriatric care experience is highly desired.
  • Ability to manage patient complexity and multiple clients with diverse needs
  • Ability to communicate effectively in writing and verbally.
  • Demonstrated ability to perform multiple concurrent tasks with minimal supervision and meet deadlines.
  • Ability to work in a fast-paced, dynamic environment and work well with others on a team.
  • Proficient in computer skills including Microsoft Office Suite (Outlook, Excel, PowerPoint, Word) and EMR experience.
  • Reliable internet connectivity required
  • Knowledge and ability to navigate internet-based tools and applications, and proficient in computer documentation
  • Demonstrates high level of professionalism.

Base Salary/ Wage Range $85,000 to $105,000 plus annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
