The Opportunity:

Limitlessli is seeking a dedicated Medical Assistant or Registered Nurse (RN) to support healthcare providers in delivering high-quality, preventive care through Annual Wellness Visits (AWVs) and Remote Patient Monitoring (RPM) programs. This remote position plays a critical role in enhancing patient outcomes by driving engagement, supporting chronic care initiatives, and ensuring thorough and accurate data collection and documentation.

This position is remote and offers you the flexibility of working from home.

Key Responsibilities:
Annual Wellness Visits (AWVs):
  • Conduct structured patient interviews to collect comprehensive health histories, risk factors, and lifestyle information.
  • Assist in developing individualized prevention plans in alignment with clinical guidelines.
  • Accurately document patient information in Electronic Health Records (EHR) systems, ensuring completeness and compliance.
Remote Patient Monitoring (RPM):
  • Educate and onboard patients to use remote monitoring devices effectively.
  • Review and track patient-generated health data, identifying trends or anomalies.
  • Engage with patients regularly to promote adherence and escalate clinical concerns as appropriate.
Qualifications:
  • Certified Medical Assistant (CMA), Licensed Registered Nurse (RN), or equivalent clinical credentials.
  • Experience with EHR platforms; eClinicalWorks (eCW) preferred.
  • Strong interpersonal and communication skills, with a patient-centered approach.
  • Detail-oriented with the ability to manage multiple patient interactions efficiently.
  • Prior experience in chronic care management or remote healthcare services is a plus.
Work Environment & Requirements:
  • Flexible scheduling to accommodate patient engagement across time zones.
  • Must adhere to HIPAA and all applicable healthcare privacy and documentation standards.
Service Fee:

Industry standards applicable to the state depending on work experience and level of expertise.

Hours:

US Eastern Standard Time (EST) – Flexible
