Humana Gold Plus Integrated is seeking an Associate Director of Care Management Support who directs and implements system of care supports for Medicareeligible adults with complex social needs in Illinois. This role focuses on building and sustaining a coordinated network of nonclinical services including peer support, community health workers (CHWs), housing navigation, and other communitybased resources to improve quality of life, independence, and health equity.

The Associate Director plays a key role in integrating care management extenders into a personcentered model that addresses social determinants of health through traumainformed, culturally responsive, and collaborative approaches. This position supports the development of workflows, partnerships, and training that enable care teams to connect members with the right resources at the right time, ensuring social needs are addressed as part of wholeperson care.

The Associate Director of Care Management Support roles and responsibilities include:

  • Lead the development of a care management model that leverages extenders (e.g., CHWs, peer supports, housing advocates) to address social determinants of health for Medicareeligible adults.
  • Collaborate with internal teams and external partners to ensure seamless integration of nonclinical supports into care planning and service delivery.
  • Build and maintain partnerships with communitybased organizations, aging services, housing providers, and public agencies.
  • Promote culturally responsive, traumainformed, and personcentered approaches across all care management activities.
  • Monitor program performance and use data to evaluate impact, identify gaps, and drive continuous improvement.
  • Develop and maintain policies and procedures that support consistent, highquality service delivery across the system of care.
  • Support training and capacitybuilding efforts for care management extenders, including CHWs and peer specialists.
  • Participate in strategic planning and contribute to the organization’s mission of advancing health equity and reducing disparities.
  • Assist in resolving individual member issues related to housing, food insecurity, transportation, and other social needs.
  • Represent the care management program in collaborative initiatives, advisory groups, and community forums.
  • Assure compliance with the required standards as identified contract, state requirementsfederal regulations.
  • Achieve yearly financial objectives through planning, directing, controlling, implementing evaluating, monitoring, and forecasting as needed to achieve budgets and cost of operations.
  • Conceive, research, plan, target, and control reductions in cost on existing product lines.
  • Work collaboratively and assist in coordinating effort between support departments within the organization.
  • Create a productive department through written and verbal communication.
  • Coordinate needed support to operations areas through smooth workflow and cost efficient, quality product delivery.
  • Continuously improve customer satisfaction through effective program monitoring to achieve timely and appropriate service delivery and reduced member problems.
  • Participate as a member of the management team in promoting Humana’s mission for strategic growth and development.
  • Represent the care management program in collaborative initiatives, advisory groups, and community forums.
  • Assist in resolving individual member issues related to housing, food insecurity, transportation, and other social needs.
  • Attend meetings as requested.
  • Fully participate in Humana’s Compliance Program, including compliance with Humana’s Code of Conduct, policies and procedures, and all applicable Privacy and Security laws.

      Required Qualifications

      • Must live in the State of Illinois. This is a Hybrid Home position with up to a 35% travel requirement within the state.
      • Bachelor’s degree in Social Work, Public Health, Human Services, or related field.
      • 6+ years of professional experience, including experience in Medicare, aging services, or communitybased care.
      • 2+ years of management or supervisory experience.
      • Strong understanding of care management models and the role of extenders in addressing social needs.
      • Demonstrated ability to lead crossfunctional initiatives and collaborate with external partners.
      • Strong, demonstrated communication, analytical, problem solving and team playing skills.
      • Ability to operate independently and in a team environment.
      • Progressive business consulting andor operational leadership experience.
      • Knowledge of MedicaidMedicare, and longterm care guidelines, benefits and policies and procedures.
      • Working knowledge of MIS systems to improve operational deficiencies.
      • Intermediate to advanced computer skills and knowledge of Microsoft Suite applications, specifically Outlook, Word, Excel, and PowerPoint.
      • Experience with data analysis, understanding, communicating, and reporting on operational trends and gaps.
      • This role is a part of Humana’s Driver Safety program and therefore requires an individual to have a valid state driver’s license and proof of personal vehicle liability insurance with at least 100,000300,000100,000 limits.
        • Preferred Qualifications

          • Advanced degree in a related field.
          • 3+ years of experience working with social determinants of health.
          • Experience managing or collaborating with community health workers, peer support, or housing programs.
          • Familiarity with Illinois Medicaid policies and systems.
            • Work at Home Requirements

              • At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is recommended; wireless, wired cable or DSL connection is suggested.
              • Satellite, cellular and microwave connection can be used only if approved by leadership.
              • Associates who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a biweekly payment for their internet expense.
              • Humana will provide Home or Hybrid HomeOffice associates with telephone equipment appropriate to meet the business requirements for their positionjob.
              • Work from a dedicated space lacking ongoing interruptions to protect member PHI HIPAA information.

                  • Direct Reports: Up to 10 associates.
                  • Workstyle: Hybrid Home; must live in the State of Illinois.
                  • Travel: Up to 35% within the state of Illinois.
                  • Core Workdays & Hours: Typically, 85 pm Monday – Friday Central Standard Time (CST) with flexible scheduling (i.e. occasional nights and weekends).

                      The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


                      $104,400 $143,600 per year


                      This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company andor individual performance.
