Licensed Practical Vocational Nurse Clinical Coordinator Hourly

    • As a LPNLVN Clinical Coordinator, you’ll contribute to our success in the following ways:

      • Reviews and clears HCHB coordination notes and processes administrative task workflows in an accurate and timely manner.
      • Obtains orders from physicians, including verbal, written and faxed orders, and enters into appropriate system for CTM review and approval.
      • Monitors patient hospitalization list, confirms status, and coordinates appropriate resumption of care.
      • Discharges patients in inpatient facility at end of episode (EOE)
      • Obtains patientrelated attachment renewal.
      • Participate in HighRisk Meetings and ensure that the best provision of services is offered.
      • Available to provide direct care, as needed.
          • Answers telephone calls from patients, patient representatives, and medical professionals and communicate messages to the appropriate party or parties.
          • Provides clerical support to patients such as but not limited to scheduling followup appointments for patients (as requested), providing health summaries to case managers, obtaining preauthorizations, and addressing patient questions or concerns.
            • What is Required?

              • Graduate of an accredited school of professional nursing
              • Current Certified Licensed Vocational Nurse or Licensed Practical Nurse
              • 1 year of home care or hospice experience
              • Able to seek patient feedback, advocate for patients, understanding of patient needs, and managing patient expectations
                  • Able and willing to travel up to 50% as needed to support business and patient needs.
                      • Sit, stand, bend, lift and move intermittently and be able to lift 50100 lbs.
                          • Valid driver’s license and insurance and reliable transportation to perform job tasks
                            • You will report to the Clinical Team Manager.
