About this role

As a Field Outreach Specialist, you will be Waymark’s frontline presence in the community, often our first point of contact with our patients, and a key member of a startup team providing feedback to help refine our workflows and build strong relationships with patients, community-based organizations, hospital staff, and other community providers. You will work to improve the health of Medicaid patients from low-income communities by engaging them where they are and connecting them with Waymark’s services and broader care delivery team. You will be part of a care team that is multidisciplinary and includes Licensed Clinical Social Workers, Pharmacists, Care Coordinators, and Community Health Workers.

Responsibilities

  • Attend in-person and virtual team meetings and trainings, including a 2-3-week long paid onboarding training program.
  • Engage individuals eligible for Waymark support in the community, in the home, and in hospitals and other healthcare facilities to introduce them to Waymark support, assess their needs, motivate patients to engage in care, facilitate connections to healthcare and social resources, and support referrals to care team members. 
  • Work from or near key local hospitals multiple days a week to connect patients to support post-discharge and reach hard-to-reach patients by meeting them where they are – meeting in the field, establishing trust, and connecting them to the broader Waymark care team.
  • Foster trust and improve patient engagement rates through effective, empathetic, and credible patient communication, including via cold outreach to individuals not yet familiar with Waymark.
  • Build and manage positive working relationships with staff at community organizations, community events, hospitals, and other healthcare facilities.
  • Support telephonic outreach to individuals eligible for Waymark support by phone/text when in-person outreach is not feasible.
  • Work with other care team members to update and maintain a directory of community resources (e.g., food banks, housing assistance programs, childcare resources, etc.).
  • Navigate technology systems to document each patient encounter in detail and accurately.
  • Participate in weekly multidisciplinary care team huddles.
  • This is a hybrid role, with a majority of time (up to 80%) spent working in the field in Snohomish and King Counties, while also involving remote work from home, as needed.

Minimum qualifications

  • Deep commitment to improving healthcare for underserved populations and a desire to help others.Highly organized and self-motivated to work independently and manage schedules efficiently.Sound judgment and the ability to quickly analyze situations.Cultural competency and ability to work with a diverse community in an empathetic, passionate and professional manner.​​Friendly, energetic, and enthusiastic personality.
  • Experience conducting home visits/”door knocks” and other field-based outreach
  • Experience in care coordination, patient outreach or working in a healthcare delivery setting.
  • Excellent interpersonal communication skills and active listening abilities.Computer literate with experience and comfort navigating a variety of technology platforms for virtual communication, scheduling, and documentation, including Google Workspace. Comfortable with ambiguity and taking on a variety of tasks as needed.Reside within a commutable distance of Snohomish County.Travel required within Snohomish and King Counties (up to 80%).Current Driver’s license and access to an insured vehicle.

Preferred qualifications 

  • Long time resident of the Puget Sound region and knowledgeable of local community resources and Medicaid populations.
  • Experience working with populations with behavioral health, social, and physical health needs.
  • Customer service and/or client services experience.
  • Multilingual skills a plus.
Hourly Rate Range

$26.54 - $30.13
